Investigation And Analysis Of The Status And Influence Factors On The Early Rehabilitation Treatment Compliance For Hospitalized Stroke Patients

Objective:To investigate the compliance of hospitalized stroke patients’ with early rehabilitation treatment and further analyze the relevant factors and discuss the appropriate measures, so as to take positive and effective measures, reduce the incidence of non-compliance, and help patients to achieve the best rehabilitation treatment effects and reduce disability rate.Methods:263 cases of cerebral apoplexy patients who have been treated in our neurology department between October 2013 and June 2014 were selected as the research objects,and all of them were specific groups from the countryside of remote areas. All the cases were consistent with diagnostic criteria of the national academic meeting for cerebrovascular disease in 1995, and were also diagnosed by head CT or MRI as new cerebral apoplexy. All the cases regained consciousness within 4-7 days after onset of cerebral infarction or 10-14 days after onset of cerebral hemorrhage(GCS>8). They were in a stable vital sign, without diseases of important organs(heart, liver, kidney,etc.). Patients with mental disorder or did not actively cooperate with the treatment were excluded. With social cognitive theory(SCT) as the theoretical basis, relevant factors affecting compliance of hospitalized stroke patients with early rehabilitation treatment were collected, including personal factors(generally condition, with/without a mate, knowledge, attitude, psychological status, the ability of daily life and physical function state), environmental factors(economic factors, the number of main care givers, family condition), social factors(social support) and so on. Compliance of hospitalized stroke patients with early rehabilitation treatment was divided into 3degrees: full compliance: patients can actively cooperate with early rehabilitation treatment, and insist till the end of the rehabilitation treatment; partial compliance:patients can accept early rehabilitation therapy, but cannot complete full rehabilitation treatment namely they will drop out during the period; noncompliance: patients refuse to accept the early rehabilitation. 263 cases of patients were given investigation with self-made questionnaire and general rating scales. SPSS17.0 was used to analyze the obtained date, to analyze the compliance of hospitalized stroke patients with early rehabilitation treatment and the influencing factors.Results:1. 263 cases of patients were given corresponding questionnaire survey, 261 effective questionnaires were recycled, which accounts for 99.24%.2. 47.89% of the hospitalized stroke patients had 3 or more symptom. For daily life ability, 36.02% of the patients had moderate dysfunction, 17.62% with severe dysfunction and 47.13% with depression.3. Average scores of relevant knowledge were 13.17±5.52 and the average awareness rate was 48.8%. The maximum scores were seen in knowledge related to strokes recurrence, while the minimum scores were in basic knowledge and knowledge related to rehabilitation. Average scores of attitudes towards stroke were(30.96±5.76).The highest score was got in attitudes towards the control of stroke’s risk factors while the lowest score was about health responsibility attitude.4. For scores of family environment, low mark accounted for 15.33%, medium mark accounted for 56.70% and high mark accounted for 27.97%. Average social support scores were 27.98±6.57, which were lower than that of the normal people(34.56±3.73).5. Among 261 research subjects, 134 patients showed full compliance,accounting for 51.34%; 70 patients showed partial compliance, accounting for 26.82%;and 57 patients showed non-compliance, accounting for 21.84%.6. Univariate analysis on factors affecting compliance of hospitalized stroke patients with early rehabilitation treatment showed that: age, with/without a mate,educational level, knowledge, attitude, mental status, the ability of daily life, physical functional status, family relationships, family support, and social support were related to compliance of hospitalized stroke patients with early rehabilitation treatment(P<0.05). But gender, occupation, income, the number of main caregivers were not associated with the compliance of early rehabilitation treatment(P>0.05). Multivariate analysis showed that: age, with/without a mate, educational level, knowledge, attitude,mental status, the ability of daily life, physical functional status, family relationships,family support, and social support were influencing factors on compliance of hospitalized stroke patients with early rehabilitation treatment.Conclusions:1. Hospitalized stroke patients often have low daily life ability, accompanied with depression and other psychological problems; their awareness on stroke relevantknowledge is generally lower, but their attitude to the disease is fair; most of the hospitalized stroke patients have good family environment, generally equal to or above the average level, but the social support is relatively low.2. Compliance of hospitalized stroke patients with early rehabilitation treatment is not optimistic in primary hospital.3. There is no correlation between age and profession with compliance of hospitalized stroke patients with early rehabilitation treatment; age, with/without a mate, educational level, knowledge, attitude, depression, and the ability of daily life,physical functional status, family relationships, and social support are influencing factors of the compliance of hospitalized stroke patients with early rehabilitation treatment. For younger patients with higher education level, having a spouse, knowing more knowledge, having more positive attitude, no depression, and better ability of daily life, the compliance of rehabilitation treatment is better.4. There is no correlation between income and the number of main caregivers with compliance of hospitalized stroke patients with early rehabilitation treatment; and patients with better family environment have better compliance of rehabilitation treatment.5. Patients with higher social support have better compliance of rehabilitation treatment.
