Objective: To compare the clinical curative effect between the clavicle hook plate and anatomic locking plate in treatment of the lateral end of clavicle NeerⅡ type fracture.Methods: 48 patient of fresh clavicle NeerⅡ type fractures were treated by surgical treatment from June 2009 to August 2012, 27 cases using clavicle hook plate,including 17 male and 10 female,with an average age of 40.3 years;21 cases using distal clavicle anatomic locking plate fixation,including 14 male and 7 female,with an average age of 38.6 years.All cases were followed-up with x-rays at 8 weeks、2 weeks、6 months、1 year after surgery and the day before removing the implants,detailed recording shoulderpain,shoulder function,imaging data and complications.Evaluated the shoulder joint function and scored by the UCLA shoulder joint function assessment scale before and after removing the implants.Results: 48 cases were followed-up postoperative from 9 months to 18 months,with an average of 16 months.In addition to 1 case of the hook plate group delayed fracture healing,other cases reached the fractures clinical healing within 12 weeks after surgery.Statistical significance was revealed by no difference of fracture healing time to the different surgical methods(p>0.05).Scores of hook plate group were lower than locking plate group before removing the implants(p<0.05),In the shoulder pain、daily activities、physical activities、satisfaction and total UCLA scores(p<0.05);after removing implants,the scores of hook plate group improved significantly comparing the scores before removing the implants(p<0.05);Statistical significance was revealed by no difference of the scores to the different groups after removing implants(p>0.05). In terms of strength, Statistical significance was revealed by no difference of the scores to the different groups before and after removing implants.Conclusion: Clavicular hook plate fixation and anatomic locking plate fixation were an effective surgical internal fixation of distal clavicle fractures. Of the hook plate,Shoulder pain、limits of activity and function may be worse than the lock plate.Locking plate fixation does not affect the acromioclavicular joints,shoulder peak and the rotator cuff. Anatomic locking plate internal fixation can be used as a preferred method of treatment for the lateral end of clavicle NeerⅡ type fracture.
