| Objectives:1.To revise the Family Status Scale in the 3rd Edition of 《Nursing Outcome Classification 》 in a localized manner, to make up the Family Status Scale of hospitalized cancer patients meeting with current needs of China and to improve the evaluation tools for nursing efficacy.2.To get a detailed understanding of care outcome situation of hospitalized cancer patients’ Family Status Scale, for the purpose of providing valuable information for the improvement of care effectiveness and efficiency, as well as for the construction and development of cancer care.3.To promote and implement the Nursing Outcome Classification, which is a standard language of international nursing, in order to bring the nursing in China more in line with that of the world.Methods:1.To integrate the items of patients’ family status in the Chinese literal translation of the 3rd edition of 《Nursing Outcome Classification》 under the authority of the original authors, and to form an item pool in initial scale in combination with the investigation of domestic and foreign researchers, through in-depth interview.2.To make predictive scales by increasing, deleting and modifying the scale items, through two-round Delphi method.3.Using random sampling method, to carry out a pre-survey of hospitalized cancer patients with modified and self-made general questionnaire and to test its reliability and validity.4.Using convenient sampling method, to perform a formal investigation towards the patients from five different 3A-level hospitals of Shanxi Province and record all the survey data into the database, against which to evaluate its reliability with the Cronbach ’s alpha coefficient, the split-half reliability and test-retest reliability, to measure its validity of scale upon Delphi method, to investigate its structure validity through exploratory factor analysis and confirmatory factor analysis, and to analyze the different impact factors on the family status of hospitalized cancer patients in parallel.Results:1. The screening of the item poolOn the basis of the Chinese literal translation of the 3rd edition of 《 Nursing Outcome Classification》, the item pool of the Family Status Scale of hospitalized cancer patients was formed through the depth interview, the literature review, etc. The prediction scale including 5 dimensions and 53 entries was obtained upon two-round Delphi method.2.Scale ReliabilityCronbach’s alpha coefficient of the total scale is 0.942, and those of each dimension are between 0.650-0.872, which display good internal consistency. The split-half reliability of total Scale is 0.771,and those of each dimension are between0.625—0.714. The test-retest reliability of total Scale is 0.867, and those of each dimension are between 0.784—0.852.3.Scale validityContent Validity: the CVI of such scale is 0.931, according to the evaluation of the panel, and the CVI of all entries is higher than 0.800.Structure validity: KM0 = 0.667, chi-square value is 1461.844, the degree of freedom528, P < 0.001. All these results are showed to be suitable for further factor analysis.Finally, five common factors are extracted, and the cumulative proportion is 74.45%.4.The factors of population sociology which may affect the Family Status Scaleof hospitalized patients of malignant tumor include:(1)Main factors which affect family coping are: educational background, age, the type of payment for medical expenses, the relationship between caregiver and patient,educational background, job of the caregiver, number of caregivers, situation of children, income per head.(2)Main factors which affect family function are: educational background, age,job, the type of payment for medical expenses, educational background, job of the caregiver, number of caregivers, situation of children, income per head.(3)Main factors which affect family economic and health status are: educational background, the type of payment for medical expenses, the relationship between the main caregiver and patient, educational background of the caregiver, income per head.(4)Main factors which affect professional care with family participation are:educational background of the patient, the type of payment for medical expenses,educational background of main caregiver, income per head.(5)Main factors which affect family’s support during treatment are: age and job of the patient, educational background of main caregiver, income per head.Conclusion:1.In summary, we have initially drawn up a scale of hospitalized patients of malignant tumor containing 5 dimensions and 53 items2.After examination, the scale was proved to have good reliability and validity,which is simple to handle and facilitate its application in clinical practice.3.The theoretical structure obtained after data analysis is mostly consistent with the 5 dimensions of the original design, and the fitting index have reached the standard of goodness.4.Main factors which affect the family status scale of hospitalized patients of malignant tumor conclude:Educational background, age, job, the type of payment for medical expenses of the patient, the relationship between caregiver and patient, educational background, job ofthe caregiver, length of time, number of caregivers, situation of children, etc.5.The main innovation:(1)In China, this study is the first to combine the nursing outcome classification and family status scale of hospitalized cancer patients, which carry on science and rigorous cross-cultural revision to make up the Family Status Scale of hospitalized cancer patients meeting with current needs of China and to test reliability and validity analysis.(2)Family Status Scale of hospitalized cancer patients based on nursing outcome classification applies for the clinical in order to promote the development of the nursing profession and bring the nursing in China more in line with that of the world.6.The Imagine of continue to carry out this studyThe samples of this study extracted the hospitalized cancer patients that just come from five hospitals of the third Level of Shanxi, therefore, in future studies, we should increase the number of samples and across regional,choose representative research objects to provide a good measuring tool for further research of family status scale of hospitalized cancer patients. |
