Objective:We created the whole-ranged nursing management model of newly diagnosed and the early stage type 2 diabetes mellitus patients based on“ standardization management of diabetes clinic based on the Omaha system-- diabetes mellitus disease management--patients post-discharge self management” to explore the effect on blood glucose, the quality of life and the levels of knowledge of type 2 diabetes mellitus patients.Methods:80 T2 DM patients were assigned into two groups. In experimental group, patients received routine outpatient care plus the management guided by whole-ranged nursing management model;In control group,patients were given routine outpatient care only. Patients’ blood glucose,quality of life and the levels of knowledge were assessed before the intervention and 6 months after the follow-up.Results:After the intervention for 6 months, Patients’ blood glucose, quality of life and the levels of knowledge in experimental group were obviously better than those in control group(P<0.01),they were statistically significant different in the two groups. In the follow-up visit after 6 months, fasting blood glucose in the intervention group was(7.792±0.922)mmol/L,postprandial 2h blood glucose in the intervention group was(9.980±1.441),fasting blood glucose in the control group was(9.042±1.557)mmol/L,postprandial 2h blood glucose in the control group was(12.275±2.113).Theindices in the intervention group were significantly better those in the control group, P<0.01.Conclusions:Omaha system can promote nursing assessment and the implementation of integrated care. The system has clinical significance in nursing work of patients with chronic diseases. Apply the whole-ranged nursing management model to manage patients can improve their blood glucose level, improve patient’s quality of life and the levels of knowledge. |
