The Intervention Effect Of Intervention On Elderly Of Housebound Based On Omaha System

Objective Discussing on the effect of application of Omaha system intervention on elderly housebound, to provide reference for the intervention of community elderly housebound.Method In 2013 July to 2013 October, 159 residential community in Tangshan City and363 residential communities in Baoding city were selected by cluster sampling,separately from Tangshan Diaoyutai community and Baoding Chinese area. Housebound screening for 1359 elderly people in the two communities in housebound, were found in215 elderly housebound people. As the research object to the 215 housebound elderly,from the arbitrary random number to a row in the table a digital continuous copy number215, 215 patients met the inclusion criteria of housebound according to the sequence of the elderly home label, random number corresponding to the singular into intervention group,even into the control group, the intervention group of 107 people, 108 people in the control group. The intervention group according to the Omaha system intervention from health education, treatment procedures, case management and supervision of four aspects of 1time a week, for a period of 6 months of intervention. The control group was the blank control. The intervention evaluation are achieved respectively using Omaha evaluation,ADL scale, GDS scale, SSRS, WHOQOL-100 with 0,6 month. SPSS13.0 software is utilized for data analysis, t test is applied for measurement data and χ2 inspection is used for count data.Results Before the intervention, compared two groups of elderly Omaha evaluation system of four major areas of status, ADL status, problem scores of depression, quality of life and social support level had no significant difference(P < 0.05). 1 Intervention and control groups to the Omaha system problem: After the intervention, In the field of environmental protection, in addition to the income question, the patients in the intervention group, health, home environment and neighborhood safety scores were better than the control group(P < 0.05). The problems in the social psychology, intervention group in the field of scores were better than the control group, the difference was statistically significant(P < 0.05). In the physical realm, except for the neuromuscular skeletal function, the patients in the intervention group hearing, vision, pain, breathing,circulation of each item score were better than the control group(P < 0.05). Patients in the intervention group of health behavior in addition to sleep / rest, nutrition, physical activity,self care, drug treatment, scores ofsupervision of health care were better than the control group(P < 0.05). 2 Intervention and control groups to ADL and quality of life status: After the intervention, compared with the control group, the intervention group of the elderly has improved ADL function, and the number of dysfunction is less than that of the control group, and the quality of life score and the each dimension scores were better than the control group, the difference was statistically significant.(P < 0.05). 3 Intervention and control groups to depression and social support status: After the six months intervention,compared with the control group, the intervention group of the elderly has improved depressive symptoms, and the number of depressive symptoms is less than that of the control group, the intervention group of the elderly social support score, subjective support and support utilization degree scores on the dimension scores were higher than those in the control group, the difference was statistically significant(P < 0.05), but the implementation of the intervention system has no significant effect on objective supportscore(P > 0.05), the difference was statistically significant.(P < 0.05).Conclusions 1 The intervention application that based on Omaha system can improve the housebound patients situation in the aspect of environment field, social psychology field, physical field and health behavior field. 2 The application of Omaha system intervention, can improve the ADL status of the elderly, but also improve the quality of life.2 The intervention program established by Omaha system can improve the social support of the elderly, but also can reduce depression.
