| Background:As one of the three major malignant tumors, ovarian cancer is the fifth largest tumor causing death in female. Most of the patients with ovarian cancer at the time of care are in 1 gynecology, ovarian cancer, the fifth largest tumor, is the leading cause of deat ate stage (stage III and IV). Although accepting both cytoreductive (debulking) surgery and platinum-based adjuvant chemotherapy after operation can improve the survival rate, the recurrence rate of patients is as high as 80% within five years. The role of adiponectin receptors (AdipoRs) during the occurrence and development of tumor has become a hot spot of research. Adiponectin (APN) has pleiotropic anticancer fat factor, while the strength of the APN antitumor activity is decided by the expression level of the organization of AdipoRs. This study is aimed at providing reference for prognosis judging and interventions via the research of the expression level of adiponectin receptor 1 (AdipoRl) in the tissue of epithelial ovarian cancer (EOC) and the relationship between AdipoRl and clinical pathology of EOC. Objective:To study the expressions of AdipoRl in the tissue of epithelial ovarian cancer (EOC), and the relationship between AdipoRl and the clinical pathological features of EOC, and to explore the Expression and Significance of AdioR1 in the EOC patients. Methods:Expressions of AdipoR] in 73 EOC patients were detected by using immunohistochemical method and clinical data of these 73 cases were retrospectively analyzed. SPSS software was used to assist in processing experimental data and analyzing the positive expression of AdipoRl in EOC tissue and the clinically relevant parameters in patients with ovarian cancer, such as the relationships among the clinical stage, pathological type, degree of cancer differentiation, ascites, diabetes, body mass index (BM1) and blood platelet count and platinum-free-intervals. Related analysis was also performed on the progression free survival (PFS) and overall survival (OS) that affected EOC patients. Results:1. Positive expression rate of AdipoRl in EOC tissue was 69.86%(51/73), no significant difference was found compared with 73.3% of the normal ovarian tissue(11/15)(p>0.05).2. The expression of AdipoRl in the EOC tissue was obviously connected with the FIGO stage (χ2=5.477, p=0.019); having ascites for initial symptoms or not (χ2=4.651,p=0.031),and the platinum-free-intervals(χ2=12.969,p=0.000). However, significant correlation was not found in patients’pathological type, degree of cancer differentiation, diabetes, preoperative BM1, postoperative BMI and blood platelet count (p> 0.05).3. For PFS, single factor analysis showed that median PFS for patients with positive expression of AdipoRl is apparently higher than those with the negative(26.6 months vs 7.4 months,p=0.000); median PFS for patients in phase â… ï½žâ…¡is 31.0 months, which is obviously higher than the 12.1 months for patients in stage Ⅲ~Ⅳ(p=0.000); median PFS for patients with ascites being the initial symptoms is 10.3 months, significantly lower than the 33.2 months of those without ascites (p=0.000); median PFS for EOC patients with diabetes is 10.3 months, while those without diabetes 17.6 months (p=0.019); median PFS for patients with their blood platelet count≥350 X 109/Lis 11.9 months whereas those with the count<350×109/L is 26.6 months(p=0.026); the PFS of the EOC patients was significantly affected by the expression of AdipoRl, FIGO stage, ascites, diabetes, and blood platelet count; whereas multiple factor analysis revealed that AdipoRl expression, FIGO stage and ascites were the independent prognostic factors for the PFS of EOC patients.4. As for OS, single factor analysis showed that median OS for patients with positive expression of AdipoRl is apparently higher than those with the negative(60.2 months vs 23.5 months,p=0.000); median OS for patients in phase â… ï½žâ…¡ is 60.2 months, significantly higher than the 38.7 months for patients in stageⅢ~Ⅳ(p=0.038); median OS for patients with ascites being the initial symptoms is 29.7 months, significantly lower than the 60.2 months of those without ascites(p=0.010); median OS for EOC patients with diabetes is 29.7 months, while those without diabetes 57.7 months (p=0.022); the OS of the EOC patients was significantly affected by the expression of AdipoRl, FIGO stage, ascites and diabetes; whereas multiple factor analysis revealed that AdipoRl expression was the independent prognostic factors for the OS of EOC patients.Conclusion:1. Positive expression of AdipoRl in the tissue of EOC patients is associated with FIGO stage, patients having ascites or not and the platinum-free-intervals, which shows that AdipoRl can become the index judging the platinum-free-intervals for EOC patients.2. AdipoRl is the independent prognostic factor for the PFS and OS of EOC patients, which can become a prognostically evaluating factor of EOC patients. |
