Application Of Self-care Theory Of Orem In Senile Patients Undergoing Artificial Total Hip Replacement

Objective：To develop a nursing intervention program which is designing with Orem self-care theory as thecore and nursing in elderly patients undergoing total hip replacement. To investigate the effect of Oremself-care theory in the treatment of senile hip joint replacement.Methods：100elderly patients of total hip arthroplasty that meet the inclusion criteria were randomlydivided into experimental group and control group with50cases in each group. The two groups had nosignificant difference (p>0.05) in gender, age, disease constitution, culture degree and operation method.The control group was given regular nursing, including peri-operation period nursing, psychologicalnursing, complication nursing and rehabilitation nursing. The experimental group was given Orem self-caretheory based on regular nursing. Different nursing and health education was implementing according to thehealth needs and the course of disease in different patients, Including wholly compensatory nursing, partlycompensatory nursing and supportive-educative nursing.The SPSS17.0statistical software package wasusing to analyzing the data and compared the index score between the experimental group and the controlgroup.Results：Compared with those in control group, The complication rate of retention of urine, lowerextremity venous thrombosis and infection is much lower than comparing group, there is significantstatistics meanings(p<0.01); The ADL scores, satisfaction and Harris score was significantly higher than thecontrol group, there is statistics meanings(p<0.05).Conclusions： Applying the Orem self-care mode in senile patients undergoing artificial total hipreplacement could reduce the complication rate, improve self-care ability and improve the function of hipjoint. At the same time, it could promote the communication between nurses and patients, improve nursingsatisfaction and achieve a good nursing effect.
