Objective:To evaluate the success rate and motor outcomes of monocular horizontal muscle surgery with extraordinary gauge in patients with concomitant strabismus.Methods:43 patients with concomitant strabismus with complete date were undertaken surgery with extraordinary amount of operation in department of ophthalmology for the First Affiliated Hospital of Guangxi Medical University between January 2009 and July 2014. strabismus and a history of previous ocular muscle operation were excluded. The 13 cases of patients with concomitant external strabismus, including perceptual exotropia in 12 cases,who underwent unilateral rectus muscle recession in 9-10mm,with medial rectus muscle excised 5-8mm in the same eye.30 patients with concomitant internal strabismus, including 22 cases of perceptual esotropia who underwent unilateral medial rectus muscle recession in 6-7mm, or/with the lateral rectus muscle resection in 6-9mm in the deviated eye ball。The follow-up time range 1.5 to 60 months。The residual deviation and duction were observed in different periods。The residual deviation below 10 PD is successful. Duction defects of-1--4 are regarded as motion abnormality.Results:34 of 43 patients were achieved good appearance(79.1%).13 patients with concomitant exotropia had outside hyperfunction before operation.9 patients had abduction defects of-1 after operation,The abducion of 1 patient was-26 weeks later after operation.8 of 30 patients with concomitant esotropia existed abduction deficiency(-1~-2) preoperative. Only 1 patient of abduction was changed from-2 to-1 and 29 patients moved normally after surgery.Conclusion:Unilateral strabismus surgery can effectively correct the concomitant squints.9-10mm of lateral rectus muscle recession can cause some patients abduction slightly restricted,6-7mm of medical rectus muscle recession has nothing to do with eyeball movement.
