The Study Of Economic Operation And Service Efficiency For Public Traditional Chinese Medicine Hospitals In County-level In Anhui Province

Objective: To the County Public Hospital of TCM(Transnational Chinese Medicine) plays an important role in the health care system, and plays a leading role in the grassroots of Chinese medicine service network, and acts as the mainstay of rural tertiary care in preventive health care network with the county general hospital. The reform of County Public Hospital of is also the key part of the reform of public hospitals. Solving the problem of development of Public Hospital of TCM is an important factor in the problem-solving of the "difficult and expensive" problem.Research Objectives. According to the description of the situation of Anhui Provincial Hospital of TCM, and the analysis of the staff in hospital classified by gender, age, education, job title, we acknowledged the basic status of the development of TCM Hospital and ownership of resources. Through the study of the status quo economy of Anhui County Public Hospital, financial compensation cases and data envelopment analysis, a comprehensive assessment of the operation and efficiency of the Hospital of comparison, we make policy recommendations for the reform and development of County Public Hospital of TCM in Anhui Province, providing a reference for policy makers to grasp the overall macro-operations around the hospital of the new medical reform, thereby forming policy recommendations to encourage development and encourage the use of TCM Hospital medicine services.Methods: This study is a longitudinal study, including quantitative and qualitative research. Quantitative survey by polling the Anhui Provincial Health Statistics Yearbook, and the collection data of authority of TCM in Anhui Province, Anhui Provincial Health Office of Planning and Finance, medical affairs and medical services and other related departments of Management Services Hospital, including the basic information resource configuration and composition of health personnel in Anhui Provincial TCM Hospital;collect the economic performance of Country Public TCM Hospital in Anhui Province: the balance of payments, savings status, liabilities, delivery of services, financial investment situation; operating efficiency situation of County Public TCM Hospital: fixed assets medical income per hundred, the average revenue per worker, the average hospital bed days per worker, outpatient and emergency visits per worker, average occupancy rate per sickbed, average length of stay for those discharged, clinic visits, emergency department visits and hospital discharge passengers; medical insurance fund income of medical revenue. Qualitative survey used structured interviews with the relevant authorities and the Hospital of TCM Institutions Management; interviews focused on the present situation of Chinese medical institutions, and forward reasonable proposals for the development of Chinese medical institutions and Chinese medicine service compensation opinions.Results:(1) Most of the TCM Hospital in Anhui Province is second-class hospital, which have less than 300 beds, and the overall size is small. TCM Practitioners(assistant) physicians accounts for 48.15% of the practicing accounting(assistant) which means the insufficient number of Chinese medicine professionals; the most health personnel in hospital is aged 25-34 year-old, which accounting for 42.48%; the main qualifications is college, accounting for 40.92%, secondary and technical education in the proportion accounted for slightly larger than a bachelor’s degree; Assistant or Physician titles accounted for 30.75%, Clerk or Disabilities titles accounted for 31.48%; staff with higher academic title is few and the health personnel structure is irrational.(2) The amount of revenue and medical services of Country Public TCM Hospital in Anhui Province increase from 2011 to 2013 including fixed assets medical income per hundred yuan, the average revenue per worker and per worker average outpatient and emergency visits and other indicators reflecting the hospital efficiency. The efficiency of operating and health care services has improved.(3) In 2013, compared with 2012, the proportion of total revenue of drug reduced 4.92%, indicating the effectiveness of early medicine reform; in drug income structure, outpatient and inpatient medicine herbal medicines and proprietary income share increases, with thereduce of western medicine. The drug revenue constitutes is more reasonable.(4) The asset-liability ratio is higher in County Public TCM Hospital of Anhui Province from 2011 to 2013, respectively, 42.14%, 51.79%, 51.88%. The development of TCM hospital is facing with the debt problems.(5) Data envelopment analysis showed that in the model 62 Country Public TCM Hospital, the overall effectively accounted for 27.42%, the technology effectively accounted for 35.48%, and scale effectively accounting 27.42%. In the current size of the hospital, the effectiveness of County Public TCM Hospital has to be improved.Conclusions: Through the analysis of County Public TCM Hospitals’ economic operation and data envelopment, we acknowledged overall economic operations of hospitals. We recommended that the government should encourage Chinese medicine reasonably according to health resources ownership status and health development plan; develop objective criteria; arrange regional planning rationally; build and improve the economic operation mechanism and compensation mechanisms. We proposed to strengthen the regional health planning of TCM hospitals; allocate regional health resources rationally; improve the quality of Chinese medicine hospital health personnel and increase the competitiveness of Chinese medicine hospital; play the role of government to invest and to protect the public welfare county public TCM hospitals; perfect the price mechanism of Chinese medicine services to encourage the formation of prices, health care, financial constraints tripartite balance aspects of development.
