| Objective Esophageal cancer is one of the primary gastrointestinal malignant tumor and it has an serious threat to human health. In recent years its incidence was decline in worldwide, but high incidence region of esophageal cancer remained at a high level. China is the country whose incidence and mortality rates of esophageal cancer are the highest in the world. In 2008, about 482,000 people in the world was found to have an esophageal cancer, and in china the new cases was 259,000. So half of the new cases was found in china. Henan province is one of the high incidence region of esophageal cancer in china, so this article retrospectively analyzed the clincial pathological data, and to found the clinical, pathological and individual influence factors of survival time of esophageal cancer surgery patients. We found the influence factors of survival time of esophageal cance can extend the survival time and improve the quality of the patients, can help the clinicians to choice the treatment for patients and also can provide help and the basis for tertiary prevention.Subjects and Methods Collected January 2007 to December 2013 in Henan Cancer Hospital for esophageal cancer radical operation patients. There were of 3309 cases was found and all the data was included in the Excel database. The statistical analysis was use the SPSS software, and survival rate was calculated use life table method. Comparison between groups using Log-Rank test, survival curve was drawn using Kaplan Meier method, multiple factor analysis using Cox model.Results This study investigated 3309 cases of esophageal cancer patients, among them 2248 were male, 1061 were female. All patients with an average age of 60.8 years old, with a median age of 61.0 years old. Survival analysis results show that the total 1, 3, 5 year survival rate was 88%, 61%, 41%. Single factor analysis showed that gender, age,pathological staging, infiltration degree, the number of lymph node metastasis, distant metastasis, cytological diagnosis, pathological morphology, tumor site, tumor length, degree of differentiation, the cut edge of tumor cells are the influence factors of survival time of postoperative esophageal cancer patients(P < 0.05). Multivariable Cox analysis showed that the pathological staging, cut edge residue, lymph node metastasis number, age, length of tumor, pathologic morphology classification has statistically significant, and also was an important independent risk factor for the esophageal cancer patients after operation(P < 0.05).Conclusions The gender, age, pathological staging, infiltration degree, the number of lymph node metastasis, distant metastasis, cytological diagnosis, pathological morphology, tumor site, tumor length, degree of differentiation, the cut edge of tumor cells are related with the prognosis of esophageal cancer patients. The pathological staging, cutting edge residue, lymph node metastasis number, age, length of tumor, pathologic morphology were independent indicators affect the prognosis of patients with esophageal cancer after operation. The pathological staging was the largest influence on the prognosis of patients with esophageal cancer.so clinicians should be under the condition of patients, select the most appropriate treatment measures and solutions, in order to improve the quality of survival, prolong life time, improve the prognosis of patients better. |
