Objective: to analyze the correlation and prognostic value between Naples prognostic score(NPS)and postoperative efficacy of elderly patients with esophageal cancer in the elderly.Through the detection of specific factors such as preoperative albumin,cholesterol,neutrophil to lymphocyte ratio(NLR)and lymphocyte to monocyte ratio(LMR),and the sorting of postoperative return visit data,To evaluate the value of Naples prognostic score(NPS)in the evaluation of surgical prognosis in elderly patients with esophageal cancer.Materials and methods: a total of 152 elderly patients with esophageal cancer in our hospital from 2014 to 2020 were retrospectively analyzed to ensure that these 152 elderly patients met the requirements of the treatment test conditions and were older than 60 years old,including 81 males and 71 females,with a median age of 72 years;Lesion location: 18 cases of upper thoracic segment,63 cases of middle thoracic segment and 71 cases of lower thoracic segment.They were divided into endoscopic group(group T,99 cases)and open group(group O,53 cases).There were 85 cases of postoperative pathological type squamous cell carcinoma(SCC),67 cases of adenocarcinoma and other pathological types.44 tumor patients had poor differentiation and 108 tumor patients had high / medium differentiation.Sort out and compare the baseline data of operation,collect and calculate the preoperative serum albumin,total cholesterol,neutrophil to lymphocyte ratio(NLR)and lymphocyte to monocyte ratio(LMR)of elderly esophageal cancer,score and grade the patients according to the NPs rule,and apply Kaplan Meier survival analysis and Cox proportional regression model for multivariate prognosis analysis,The efficacy evaluation was carried out according to who solid tumor efficacy evaluation standard 1.1(RECIST 1.1)until the patient had tumor progression or death.SPSS 20.0 statistical software was used to analyze and process the data.The counting data is expressed in percentage,and the measurement data is described in mean or median.use χ 2 test and analyze the different clinical characteristics and differences among the three groups,draw the survival curve by Kaplan Meier method,and compare the differences of PFS between the three groups by log rank II method.All tests were performed by bilateral test,with P < 0.05 as the difference,which was statistically significant.Results: The median serum albumin was 4.34(3.11-5.26),the median total cholesterol was 216(97-386),the median NLR was 2.01(0.80-5.79),and the median LMR was 4.34(0.82-9.45).1.NPS(HR: 2.360;95% confidence interval: 1.382-4.027;P = 0.002)was an independent predictor of overall survival(OS),followed by degree of differentiation(HR: 1.946;95% confidence interval: 1.260-3.005;P = 0.003).2.The degree of differentiation(HR: 1.811;95% confidence interval: 1.228-2.725;P = 0.003)and NPS(HR: 2.531;95% confidence interval: 1.511-4.242;P<0.001)were independent prognostic factors for disease-free survival(PFS).Conclusion: Naples prognostic score(NPS)is a powerful predictor of long-term prognosis in elderly patients with esophageal cancer. |
