The Investigation On The Requirements Of Community-Based Household Services For The Elderly In Tangshan City

Objective To discuss the elderly pension intentions, the state of household pensionservice requirement and its affecting factors in Tangshan communities, hope it canprovide references for consummating the pension strategies, satisfying the elderlypension intentions and developing the community elderly household pension service.Methods The cluster sampling method was used in the study. Two communities wereselected from the35communities which carried out household pension service from Oct.2012to Jan.2013, in Lunan and Lubei district.724elderly who were over60in the twocommunities were questioned face to face in this study. Household Elderly GeneralSituation Investigate Sheet, Elderly Pension Intentions and Requirement InvestigateSheet were used. The sheets included the aspects of the elder life situations, the dailyexercise abilities, the society supports and the depression situations. From the analysis ofthe choice of the elderly pension style, the requirements of the pension service and itsrelated influencing factors to get the requirements of the different elderly to householdpension service. SPSS17.0software package was used for statistical analysis.Results1The community elderly general situation evaluate result: The general healthcondition of the elderly is in a low level,98.6%of the elderly are suffering from chronicdiseases,45.3%of the elderly daily excise ability is going down, with varying degrees ofdecline; the score of Social support is lower than the national norm(t=-9.394, P＜0.05);The prevalence of depression in the elderly was18.9%.2In order to make their life haveeconomic security,53.6%of the elderly tend to choose on their own pension savings; Inthe choice of ways of pension, selecting the proportion of the pension that occupy thehome is the largest, which is accounted for31.5%; The results of Logistic regressionanalysis, in all the analysis of the variables: incomes, activities of daily living anddepressions have affected the pension mode selections(P＜0.05). lower Incoming, poorability of daily activities and depressions of the elderly have more demand of the homecare.3The analysis results of many factors affect home endowment service needs:cultural degree, the number of chronic disease, activity of daily living is the main factors ininfluencing the service demand and the difference has statistical significance(P＜0.05);the higher Cultural degree, more chronic diseases and poorer daily activities of the elderly have the more demands of the home endowment service.4Comparative study ofdifferent aged groups demand for home based care services: the elderly on health care service ofthe most urgent demand accounted for80%; Service of daily life care demand accountedfor44.3%, mainly concentrated in the elderly,who have lower cultural degree, morechronic diseases and poorer ability of daily activities; On spiritual consolation serviceaccounted for34.8%, focused on the elderly with higher Cultural degree, more chronicdiseases and poorer ability of daily activities.Conclusion1The community elderly health condition is not ideal at present and theprevalence of chronic diseases is higher; Social support is lower than the national normal.2The elderly is preferred to retirement home care.3Higher demand of communityelderly to reside endowment services, including medical and preventive health careservices is the core of the elderly home endowment service needs. The culture degree ofthe elderly, the number of chronic disease and the activity of daily living are the mainfactors in influencing the pension service.
