ObjectiveTo explore the correlation of serum sex hormone-binding globulin (SHBG) withirisin, obesity, insulin resistance, glucose-lipid metabolism in women with polycysticovary syndrome(PCOS).Methods42PCOS patients were sampled in experimental group, which were diagnosedaccording to the Rotterdam criteria,32patients with pure tubal factor and/or malefactor infertility were randomly assigned as control group. These outcomesincluding weight, height, BMI calculation, waist circumference, hip circumferenceand waist-to-hip ratio(WHR) were measured in both groups. According to thestandard of BMI≥25kg/m2for obesity, the two groups were divided into obese andnon-obese group. The following outcomes were measured in four groups:(1) earlyfollicular phase serum follicle stimulating hormone (FSH), luteinizing hormone (LH),testosterone (T), estradiol(E2), progesterone(P), prolactin(PRL);(2) determination offasting plasma glucose(FPG), fasting blood insulin(FINS), homeostasis modelassessment was applied to assess the status of insulin resistance index (HOMA-IR),The PCOS patients were divided into two groups according to HOMA-IR;(3)determination of triglyceride(TG), total cholesterol(TC), high density lipid-cholesterol(HDL-C), low density lipid-cholesterol(LDL-C);(4)the other tube retention aftercentrifugal blood serum, with the competitive enzyme immunoassay technique fordetermination of serum SHBG, irisin. Free androgen index(FAI) was calculated.Result1. The serum LH, LH/FSH, T, FAI, FINS, HOMA-IR levels in PCOSpatients were higher than that of the non-PCOS patients (p <0.05), while the serum SHBG, HDL-C, irisin levels in PCOS patients were lower than that of the non-PCOSpatients (p <0.05).2. In both PCOS patients and non-PCOS patients, the serum FINS, HOMA-IR,TG, TC, LDL-C levels in obese patients were higher than that of non-obese patients(p <0.05), the serum SHBG, HDL-C, irisin levels in obesepatients were lower than that of non-obese patients(p <0.05).3. The serum FINS, HOMA-IR levels in obese PCOS patients were higherthan that of the obese non-PCOS patients (p <0.05). The serum HDL-C, irisinlevels in obese PCOS patients were lower than that of the obese non-PCOS patients(p <0.05). The results of HDL-C and irisin were the same comparing non-obese PCOS patients with the non-obese non-PCOS patients, but the serum FINS,HOMA-IR levels did not show significant difference.4. BMI, WHR, T, FAI, TG, TC, LDL-C in PCOS patients with insulinresistance were higher than that of the PCOS patients without insulin resistance (p<0.05). While SHBG, HDL-C, irisin in PCOS patients with insulin resistance werelower than that of the PCOS patients without insulin resistance (p <0.05).5. Correlation analysis results: The level of serum SHBG was negativelycorrelated with BMI, WHR, T, HOMA-IR, TC and TG. The level of serum SHBGwas positively correlated with the level of HDL-C and irisin. And a multipleregression analysis found that the best model to appraise SHBG level included T andHOMA-IR.Conclusion1. The level of serum SHBG is decreased in women with PCOS,especially inthose with obesity and insulin resistance. It is associated with abnormality ofglucose-lipid metabolism.2. The level of serum irisin is decreased in women with PCOS. The low level ofSHBG indicates low level of irisin. Since irisin is associated with obesity andinsulin resistance, it may be a new potential target of the treatment in women withPCOS.
