The Clinical Comparison Between PVP And PKP In The Treatment Of Osteoporotic Thoracolumbar Compression Fractures

Objective:Percutaneous Vertebroplasty(PVP) and Percutaneous Kyphoplasty(PKP) are the major treatments of osteoporotic vertebral compressive fractures(OVCFs),as the minimally invasive surgical treatments with development for decades,they are through a large number of basic and clinical research. Today clinicians have different opinions about it. In order to guide clinical work, this article gives a comparative study of the clinical treatment effect and the feasibility of them.Methods:Through the collection of the43patients who had OVCFs and were treated by minimally invasive percutaneous vertebroplasty in the spinal specialist in Affiliated Hospital of Nanjing University of Chinese Medicine from2009to Nov.2013, including23cases for PVP,20cases for PKP.And compare the operation time, hospitalization time, cost of treatment, the preoperative and postoperative VAS score, restoration of the anterior and middle height of vertebral body, Cobb angle correction and postoperative complications such as bone cement leakage occurred using statistics software.Results:the mean age and hospitalization day of patients in the two groups had no significant difference,and all patients’pain were relieved,all cases’ anterior height of vertebral body,middle height of vertebral body and Cobb angle were recover, and have no significant difference between them (P>0.05), PKP have higher bone cement leakage rate than PVP,but its operation time and cost are much higher than that of PVP(P<0.01).Conclusion:(1) The main difference between the two methods is bone cement leakage rate, in this study, patients who treated with PKP had no leakage of bone cement, is lower than PVP treatment group,and reduced risk of other serious complications caused by bone cement leakage.(2) Patients who had PKP need more operation time and cost compare with PKP. China has large population base, and our economic and social development is in the primary stage of socialism,so PVP is a minimally invasive operation which is effective and relatively inexpensive,and it is worthy of promotion.
