Objective:To analyze the high risk factors associated with reintubation after planned extubation (RAP) in the Postanesthesia Care Unit(PACU).Methods:Patients, who saw RAP in the space from January1,2010to December31,2012, were retrospectively sorted out as RAP cases in the PACU of SIR Run Run SHAW Hospital,School of Medicine,Zhejiang University. Those who did not suffering from RAP during the same period drawn by proportion of1:5randomly were taken as control group.We compared RAP cases with the control group from patient factors, surgical factors and anesthetic management. Then the high risk factors were deduced and were investigated with single factor analysis and Logistic multiple regression analysis.Results:Of the46507patients,32(0.07%) sustained RAP.According to univariate analysis, the factors associate with reintubation after planned extubation (RAP) in the Postanesthesia Care Unit(PACU) were older, males, BMI(kg.m-2)≥30, higher scores of ASA physical status and those preoperative oxygen desaturated (Sat O2<95%)breathing room air. Multivariate logistic analysis indicated that the risk factors for RAP were ASA physical status III(OR=9.690,95%CI2.814-33.374),preoperative COPD(OR=9.498595%CI1.160-77.749),systemic inflammatory response syndrome (SIRS)(OR=10.770,95%CI1.450-80.012)and thoracotomy(OR=13.325,95%CI4.501-39.451).Conclusion:Based on these predictors of RAP,further studies will be prompted on preventative measures or evaluation of how to improve outcome in the PACU.
