[Objective]This subject aims to explore the feasibility of the "toxic heat" treatment to peptic ulcer and Huanglian Jiedu decoction in active peptic ulcer clinical application value and its mechanism of action by clinical and experimental research.[Method]Clinical researchMethods:Out-Patients from December2011to January2013which are conformed to the selected standard,were treat with the modified Huanglian Jiedu Decoction. The main observation include clincal symptoms and adverse reaction.Experiment researchMethods:90healthy SD. rats were used to accomplish the gastric ulcer heat model by acetic acid, and they were randomly divided into six groups with each group of15only, blank group and model group were given saline lavage; Ranitidine group was given western medicine lavage; Chinese medicine groups was given each dose lavage Huanglian Jiedu decoction,1/d, for2weeks. To collect the serum and gastric tissue after the end of research,with Enzyme-linked immunoassay to detect expression of NO and ET in rats serum, immunohistochemical method Was applied to observe the Bcl-2expression, as well as to observe rat gastric mucosa pathological morphology.[Results]Clinical researchResults:the modified Huanglian Jiedu Decotion has good effection for heat in stomach syndrome, on the inprovement of TCM Syndrome.Experiment researchResults:(1) the naked eye observation and HE staining slice comparison:except of the blank group and model group, the rest of the group in the rat gastric mucosa pathological morphological changes alleviated in some degree, among which, Huanglian Jiedu Decoction high dose group was particularly significant;(2) NO, ET-1, compared with model group,NO expression in western medicine and Chinese medicine group are higher than model group, and ET levels were less than model group (P<0.05)(3) Bax, Bcl-2: ranitidine group, Huanglian Jiedu Decoction high dose group can reduce Bax positive expression rate and increase the Bcl-2positive expression rate, and were significant different(P<0.05)from the model group.[Conclusion]Clinical researchConclusion:The modified Huanglian Jiedu decoction can improve clinical symptoms and no adverse reaction.Experiment researchConclusion:Huanglian Jiedu Decoction can significantly improve the degree of gastric mucosal injury in rats, the mechanism study showed that Huanglian Jiedu decoction can not only increase the NO level to improve blood flow and oxygen to repair the stomach mucous membrane by the antagonism of ET-1, and restore the balance between cell proliferation and cell loss in order to promote the healing of ulcer by intervening in Bax apoptosis factors, the Bcl-2expression.
