Objective:To observe the clinical efficacy of Bilevel Positive Airway Pressure (BiPAP) non-invasive Ventilation in the treatment for chronic obstructive pulmonary disease(COPD),which complicated with pulmonary interstitial fibrosis.Methods:60patients of COPD complicated with pulmonary interstitial fibrosis during Res-piratory Failure were randomly allocated into experiment group and control group. The patients in experiment group (30cases) were treated with conventional therapy and Bi-level positive airway pressure, The control group (30cases) were treated with conventional therapy and continuous oxygen inhalation by nasal pipe. Symptoms and Vital signs and arterial blood gas parameters be-fore and after treatment as well as days of hospitalization were compared.Results:The major parametric variation of experiment group after five day therapy compared With before its therapy:The outcome measures of patients were improved significantly:RR t=10.22,p<0.05,HRt=22.4,P<0.05,PaO2t=23.4,P<0.05,paCO2t=5.88,p<0.05,There were Significant differences in the experiment group. The control group after five day therapy compared with be-fore its therapy:The outcome measures of patients were improved:RR t=6.9,p<0.05,HR t=18.83,p<0.05,PaO2t=12.6,P<0.05,PaCO2t=4.14P<0.05, differences Were statistically significant. After5day treatment, The outcome measures of patients in the experiment group were improved significantly than those in control group:RR t=-3.96,p<0.05,HRt=-5.23,p<0.05,paO2t=4.85, p<0.05,differences were statistically significant, the improvement in the treatment group by BIPAP non-invasive ventilation was much superior to symptoms and vital signs and arterial partial pres-sure of oxygen in control group with obvious statistic difference(P<0.05). The day of staying in hospital and the cases of tracheal intubation in the treatment group were less than those in control group.Conclusion:The applications of BIPAP non-invasive ventilation in chronic obstructive pul-monary disease complicated with pulmonary fibrosis can quickly improve arterial partial pressure of oxygen, clinical symptoms, respiratory rate and heart rate. In addition, BIPAP can promote re- covery from respiratory failure, BIPAP combined with conventional medical therapy was better than continuous oxygen by nasal pipe combined with conventional medical therapy.
