| ObjectiveBased on retrospective analysis on the clinical information of2145patients with breast fibroadenoma, and diagnostic analysis on thecorresponding imaging examination results, this paper further clarify theimaging antidiastole and the principle of treatment, to make a contribution tothe diagnosis and therapy.MethodMaterials of2145patients who were diagnosed as breast fibroadenomabased on postoperative pathological outcomes were collected from those whowere treated in our hospital from January2006to January2014.We classifyrecords of clinical factors, such as age, position of fibroadenoma, diameter offibroadenoma and postoperative pathological outcomes, and analyze relationalaspects statistically. At the same time, we consulted the imaging examinationresults of455subjects who had taken ultrasonic and MRI examination beforethe surgery, and made a comparison with postoperative pathological outcomes.Results1.Age distribution of2145patients2145patients are grouped according to age.26patients(1.2%)are in theage group between11and15,330patients(15.4%)are in the age group between16and20,474patients(22.1%)are in the age group between21and25,326patients(15.2%)are in the age group between26and30,307patients(14.3%)arein the age group between31and35,335patients(15.6%)are in the age groupbetween36and40,163patients (7.6%) are in the age group between41and45, 97patients (4.5%)are in the age group between46and50,47patients (2.2%)arein the age group between51and55,30patients (1.4%) are in the age groupbetween56and60,10patients (0.5%)are in the age group between61and65.2.Location distribution of5172lesions2260lesions(43.7%) of5172are in the right breast. Among them1117lesions (49.4%)are in the upper outer quadrant,345lesions(15.3%) are inthe upper inner quadrant,292lesions (12.9%)are in the lower outerquadrant,255lesions (11.3%)are in the lower inner quadrant,251lesions(11.1%) are around areola.2912lesions(56.3%) of5172are in the leftbreast. Among them1348lesions(46.3%)are in the upper outer quadrant,503lesions(17.3%) are in the upper inner quadrant,392lesions (13.5%)are inthe lower outer quadrant,368lesions (12.6%)are in the lower innerquadrant,301lesions(10.3%) are around areola.3.Correlation between the diameter of fibroadenoma and the age ofpatientsThe largest diameter of5172fibroadenoma is9cm.2145patients aregrouped according to the age,5172lesions are grouped according to theirdiameters. There is a significant difference in the diameter of fibroadenomabetween age groups(χ2=47.5,P<0.05). Most patients of giant fibroadenoma(diameter>5cm) are in the11-20age group.4.Ultrasound and magnetic in the diagnosis of breast fibroadenomaThe ultrasound image features of fibroadenoa:①shape:421lesions withregular shape,115lesions with irregular shape.②capsule like echo:392lesionswith capsule like echo,144lesions without capsule like echo.③inner echo:347lesions with uniform inner echo,189lesions without inner uniform echo.127lesions of them with calcification,62lesions of them with cystic change.④227lesions with CDFI=1,276lesions with CDFI=0,33lesions with CDFI>1. The MRI image features of fibroadenoa:①shape:438lesions with regularshape,98lesions with irregular shape.②lesion edge:411lesions with clearedge,86lesions without clear edge,39pieces with burr edge.③490lesions withequal signal in T1-weighted sequence,46lesions with low signal inT1-weighted sequence.④428lesions with high signal in T2-weightedsequence,92lesions with low signal in T2-weighted sequence,16lesions withmix signal in T2-weighted sequence.⑤354lesions with improved form foruniformity,95lesions without improved form,87lesions with improved formfor nonuniform.⑥TIC:421lesions with duration,101lesions the platformperiod,14lesions with outflow reduce.Ultrasonic diagnosis is correct in424out of455patients with breastfibroadenoma, the diagnostic accurate rate is93.2%.31patients aremisdiagnosed.9out of31patients have giant fibroadenoma.22out of31patients have fibroadenoma diameter<5cm.. Among9patients,8aremisdiagnosed as phyllodes tumor,1is misdiagnosed as breast cancer. Among22patients,13patients are misdiagnosed as breast cancer,4patients aremisdiagnosed as phyllodes tumor,2patients are misdiagnosed as breast lipoma,2patients are misdiagnosed as breast hyperplasia,1patient is misdiagnosed ashamartoma. MRI diagnosis is correct in447out of455patients with breastfibroadenoma, the diagnostic accurate rate is98.2%.8patients aremisdiagnosed. Among them3patients(fibroadenoma diameter<5cm) aremisdiagnosed as breast cancer,5patients (giant fibroadenoma)aremisdiagnosed as breast cancer. The comparison of diagnostic accuracy rates ofMRI and ultrasound is statistically significant (X2=14.2, P <0.05).5.Breast fibroadenoma with other breast diseaseAmong2145subjects investigated,115patients are with phyllode tumor,112patients are with intraductal papilloma,185patients are with abscess or acute mastitis,594patients are with cyst,38patients are with atypicalhyperplasia, and54patients are with breast cancer. The patients ofcomplicating other breast diseases are significantly different among patients invarious age groups.(χ2=277.6P<0.05).Conclusion1. Women under age30are more vulnerable to fibroadenoma, and theincidence reaches its maximum between age21-25.2. Lesions are commonly found in the upper outer quadrant of breasts.3. There is a significant difference in the size of fibroadenoma betweenvarious age groups.4. Most giant fibroadenoma(diameter>5cm) patients are in the11-20agegroup.5. MRI has an outstanding value in diagnosis and antidiastole offibroadenoma.6. Fibroadenoma can cause many complications, and the cases ofcomplicating other breast diseases are significantly different among patients invarious age groups.7.The incidence rates of breast fibroadenoma with breast cancer increaseswith age,and it is common in adults over age40. |
