Objective: This study focused on investigation of basic situation of patientswith unilateral or bilateral Carpal Tunnel Syndrome (CTS)including epidemiologyand clinical characteristics of the disease. We analyzed the frequency of risk factorhappened in these cases and aimed to get more information on the early diagnosis andearly treatment of the CTS.Method:We collected240cases of CTS from January2003to December2012,who were treated in China-Japan Union Hospital, with166unilateral cases and74bilateral cases. We retrospectively analyze clinical data from the patients includingname,age,sex, occupation, duration of hospital stays, hospital costs, residence,dominant hand, medical and family history, signs and symptoms of the disease,examine results, surgical procedure and prognosis after operation. Statistical analysiswas made by SPSS13.0software. The comparison between two groups ofmeasurement data was made by t test or rank sum test, and that between count datawas made by chi-square test.Result:①The age ranged from13to79years old with the average of49.61±11.65years old. The group of age between40to60years old had the largestproportion of66.25%. There were much more female patients (male: female,70:170).The married patients accounted for92.09%. The proportion of civilian personnel was27.92%, and that for farmers and workers is25.42%and23.33%respectively. Thenumber of urban patients was higher than that of rural patients (urban65.42%, rural34.58%).②The average hospitalization days was eight days and the basic medicalinsurance covered52.09%of all patients. There were most patients in2011. Morepatients were admitted in June and October while less from July to September.③166cases were unilateral(69.17%) and74were bilateral (30.83%). The number of patients who got preoperative treatment was9(3.75%), and60patients(25.00%) hadconcomitant disease.11patients(4.58%) had a family history of CTS and19patients(7.92%) had a history of CTS.④The main symptom of CTS is numbness (98.33%),and the second symptom is pain(72.92%). The positive proportion of Tinel signs andPhalen test reached to90.00%.75.00%of all cases took neural electrophysiologicalexamination and all patients showed positive results. Most patients(94.59%) acceptedmedian nerve neurolysis.⑤48hours postoperative evaluation showed that numbnessor pain eliminated or significantly reduced in191cases (79.59%), hand numbnessaggravated in11cases (all of them had more than96months history and8cases inthem were bilateral) and numbness or pain improved mildly in another38cases. Theone-three months postoperative follow-up showed that symptoms were completelyresolved and the function recoveries were excellent in124cases (51.67%), symptomsmostly disappeared and function recoveries were good in108cases (45.00%), andsymptoms still existed or worse with no functional recovery in8cases (3.33%).⑥There were more bilateral CTS in female patients(P=0.008), in patients with familyhistory (P<0.001)and in left hand dominated patients(P<0.001). However, themedian duration of bilateral patients was significantly longer than unilateral ones (P <0.001).Conclusion:①The onset age of CTS patients was between13-79years old,while the40-60years old age group accounted for the highest, up to66.25%. Therewere more women patients.②In this study, the urban population accounted for abigger proportion of65.42%; There were higher proportion of civilian personnel,farmers, workers than that of other occupations, among them the civilian personnelwere the most, accounting for27.92%.③The most presentation of CTS wasnumbness, accounting for27.92%, followed by pain.④The admission rate was higherin the junction of spring and summer or junction of summer and fall. The morbidityincreased year by year in this study. The basic medical insurance was the main formof payment in CTS patients.⑤The bilateral CTS accounted for35.88%of femalepatients, while18.58%of male cases. The left dominant hand population had a higher prevalence of bilateral CTS, accounting for57.14%. The patients with family historywere all bilateral.⑥The duration of bilateral patients was significantly longer thanthat of unilateral patients.
