Clinical Comparison Analysis Of Longitudinal Open Incision And Small Incision Of Distal Wrist Horizontal Stripes For Carpal Tunnel Syndrome

ObjectWrist horizontal stripes were used to place small incision and traditional palm incision treatment of carpal tunnel syndrome,according to the evaluation of hand function changes in patients with curative effect.MethodsSince September 2008 to September 2012,wrist horizontal stripes of small incision carpal tunnel release(SICTR)and open carpal tunnel release(OCTR)are used to treat 84 carpal tunnel syndrome patients in our hospital.106 hands,12 men(14%),72women(86%).Aged 27～81years,average of 53.Patients with a history of hand trauma or confounding comorbidities were excluded.Divided into two groups according to different surgical methods.Group 1 as the OCTR group,4 male and 6 hands,34 women and 40 hands.Group 2 as the SICTR group,6 men and 10 hands,40 women and 50 hands.The postoperative evaluation includes operation time,length of stay and blood-loss.The complications were observed during follow-up period.Analyzed by analysis of T test.ResultsDuring follow-up period(13～24 months),average of 11.6 months.18 patients were lost to follow-up(Group 1:6 patients,Group 2:12 patients).No wound infection occurred in the two groups.Group 1 has 7 cases of columnar pain,recurrence in 8 cases,2 cases of excessive scar hyperplasia.Group 2 has no obvious bleeding or postoperative nerve damage,but 3 cases of columnar pain,recurrence in 1 case,3 cases of paresthesia.Group 1:preoperative(2.97±0.87),postoperative(6.83±1.15).Group 2:preoperative(3.41 ±0.95),postoperative(10.11 ±0.99).No significant difference of preoperative hand function score between 2 groups(P>0.05).The postoperative hand function score of group 2 were higher than that of group 1(P<0.05).ConclusionThe classical open carpal tunnel release(OCTR)has many complications.The small incision of wrist horizontal stripes has high security,short operation time,little trauma,beauty appearance,low incidence of postoperative scar irritation,short hospitalization time,and low expense.
