| Objective: Using cross-sectional survey method, understand the communityeducation institutions at the county level people early renal damage of hypertensionepidemiological status quo, analysis to explore the related factors of early hypertensiverenal damage, at the same time on the early renal damage of hypertension TCM syndromedistribution and its correlation with urinary microalbuminuria (MAU), for the developmentsuitable for the northwest county in shandong province community will provide a scientificbasis for early renal damage of hypertension prevention and treatment strategies.Methods: In shandong province dezhou PingYuanXian education institution staff(including retirees) were investigated, using standard questionnaire survey form to collectrespondent’s gender, age, work unit, phone number, level of education, living habits,family history, hypertension history, personal history status and classification ofsyndromes and treatment of chronic quantitative data, and according to the standardizedmethod of measurement survey respondents were height, weight, waist circumference, hipcircumference, blood pressure, central arterial pressure (cSBP), brachial ankle pulse waveconduction velocity (baPWV), small artery elasticity (C1), arterial elasticity (C2), urinarymicroalbuminuria (MAU), etc., the final summary data, using chi-square test and rankcorrelation analysis on comparing the related factors of hypertension in the early renaldamage analysis; Using t test analysis of hypertension early kidney damage and renaldamage group the difference of vascular index; By unconditional Logistic regressionanalysis of the related risk factors for hypertension early renal damage, and at the sametime for early renal damage in hypertension TCM syndrome type distribution, and analysesthe effect of high blood pressure drug compliance at all levels.Results:(1)(1),787cases of simple primary hypertension patients, MAU positiverate is4.1%;(2) Related factors comparison analysis: MAU piecewise linear positivecorrelation between the positive rate and age (P <0.05); MAU positive rate and linear positive correlation between hypertension grading (P <0.05), gender, smoking, alcoholconsumption and coronary heart disease than hypertension related factors of early renaldamage;(3) Hypertension early kidney damage and renal damage group of vascular indexresearch: MAU positive C1value is lower than the negative group (P <0.05); MAUpositive baPWV value higher than negative group (P <0.05); MAU positive PP valuehigher than negative group (P <0.05).(4) Logistic regression analysis: age, pulse pressure(PP) is a risk factor for hypertension early kidney damage; Gender, body mass index,smoking, drinking alcohol is not a risk factor for hypertension early renal damage.(5) Thearea county education people suffered hypertension mainly exist KangCheng, Yin hand,phlegmy wet indicates, Yin and Yang deficiency of four kinds of syndrome, hypertensionincidence of early renal damage, respectively,3.0%,4.0%,1.5%,4.0%, P>0.05, showthat between each card type MAU positive rate difference is not statistically significant.(6)Level1, level2, level3hypertension patients long-term users accounted for45.9%,56.2%and45.9%respectively, P <0.05, suggesting that hypertension illness the person thatweigh more than the better light person adherence.Conclusion:(1) In shandong province dezhou PingYuanXian education institutionscrowd the prevalence of hypertension in the early renal damage was4.1%, lower thandomestic research level.(2) The MAU piecewise linear positive correlation between thepositive rate and age; MAU positive rate and linear positive correlation betweenhypertension grading; MAU positive group than MAU negative artery elasticity bad, MAUpositive group of large artery stiffness increases; Age, pulse pressure is a risk factor forearly renal damage induced by hypertension, hypertension patients by regular medicationprompts the county area, control of blood pressure to reduce the incidence of hypertensionin the early renal damage.(3) Surveyed the crowd disease hypertension illness the personthat weigh more than the better light person adherence, therefore, should strengthen thepropaganda education, enhance the consciousness of the local people’s "no diseaseprevention first", through the regular medication to prevent the occurrence of renal damage,or delay its progress. |
