Inappropriate hospital admission is one of the manifestations of excessive use ofhealth services. Since the implementation of the new rural cooperative medical system(NRCMS),thehospitalization rate among people in rural areas was rising from3.4%in the year of2003to8.4%inthe year of2011.There are unreasonable use among these additional inpatient services.Thisphenomenon is not only a serious waste of scarce health resources, but also posed a seriouschallenge to the management of the medical insurance fund.Paying attention to the problemof excess demand for hospital services is of great significance for the norms of health careservices utilization behavior and the healthy and sustainable development of the medicalinsurance system.ObjectiveThrough theoretical research and quo empirical surveys in the typical region tounderstand the status of inappropriate hospital admission among rural residents in the newrural cooperative medical system.And analysis the characteristics of inappropriate hospitaladmission on disease, people, institutions and flow based on medical records.Then inferunreasonable admission influencing factors with results of theoretical analysis andempirical analysis.Lastly, putting forward suggestions and measures to reduceinappropriate hospital admission.MethodsThourgh systematically scratching among domestic and foreign related research andliterature, drawing on existing research experience and results; selecting three optionalcounties in Hubei Province as samples of typical field research; applying earlier completionof the new rural cooperative medical hospitalization services excessive demandidentification standards as a tool to discriminate on medical records of some patients amongcounty and township medical institutions.Results 1.The status of inappropriate hospital admission is different in three counties.Theregion B has the highest unreasonable rate of hospitalization.2.Univariate analysis showed, influencing factors of unreasonable admitted amongcounty and township medical institutions are the same.3.Multivariate analysis showed, influencing factors of unreasonable admitted amongcounty medical institutions is different from township medical institutions.Conclusions1.Characteristics of inappropriate hospital admission: From a regional point of view,areas with high proportion of patient compensation has high unreasonably admission rate;Judging from the disease, respiratory diseases has the highest unreasonable hospitalizationrate; from the crowd distribution point of view,0-4year-old children unreasonableadmission was the most serious; from the institutional point of view, township hospitals andunreasonable admission is higher than the county-level medical institutions.2.Influencing factors of inappropriate hospital admission: From a theoretical point ofview, the new rural cooperative medical system itself will impact unreasonable admission.From the empirical research found that the patient’s age, admission departments,unreasonable admitted to hospital suffering from the disease will impact.Innovation and InsufficientAt present,domestic studys on excess examination and medication duringhospitalization are a lot.This paper is mainly focused on the inappropriate hospitaladmission and maked up for deficiencies research.
