ObjectiveTo observe the analgesic effect of dezocine combined with propofolon on artificial abortion in order to make a seasonal basis of clinical medicine.Methods200early-pregnant women were randomly divided into four groups accord-ing to randomly digital table. Patients in group A and B were respectively gi-ven fentanyl1μg/kg and2μg/kg before given propofol2.0mg/kg;Patients in gr-oup C and D were given0.1mg/kg and0.2mg/kg dezocine respectively bef-ore given propofol2.0mg/kg.anaesthesia methods:All the patients must prepare for fasting and water-deprivation for6-8hours,and are required to be inth-e lthotomy position without premedication after entering the operating room.p-uncture forearm vein,infusion Lactated Ringer’s Solution whth the speed of dro-pping10ml/kg/h,routine monitoring and record HR,DBP,SBP,MAP,SpO2,The p-atients inhalate oxygen flow is5L/min through nasal catheters,fentanyl should be injected respectively in group A and B3minutes before surgery, and dez-ocine should be injected respectively in group C and D15minutes before su-rgery,propofol2.0mg/kg should be injected within1min and one minutes bef-ore surgery,The surgery can be operated when eyelash reflex disappear,and additional propofol0.5-1mg/kg should be injected according to the changes of circulati-on and respiration.observation targetrgeneral condition of the patients,the totaldos-age of propofol,the onset time, operation time,wake up time, recovery time of ori-entation, the changes of respiratory and circulatory function,anesthetic effects,p-ostoperative analgesia,side-effects and remedial measures.Results1Ages,body weights,gestational weeks,the onset time, operation time,recove-ry time,and recovery time of orientation,ASA â… ï½žâ…¡ in the four groups had no statistical significance(p>0.05).2All patients underwent surgery successfully without anesthesia failure.3The difference of SBPã€DBPã€MAPã€HR and SpO2in four groups ha d no statistical significance pre-operation.(P>0.05).2min after intravenous inj- ection of propofol,compared with the pre-anesthesia, the change of SBPã€DBP〠MAPã€HR had significant declines in all the groups (P<0.05).compared grou p B with group Aã€C and D during intraoperative,the change of SBP,DBP,MA P,HR and SpO2had significant declines (P<0.05). There was no statistical si-gnificance between the rest of the groups(P>0.05).4The change of SBPã€DBPã€MAPã€HR in the awaking stage of anesthesia still inferior to which pre-anesthesia (P<0.05)5The total dosage of propofol in group Aand C higher than group B a-nd D.There was no statistical significance between the rest of the groups(P>0.05).6The incidence of respiratory depression in group B was the highest P<0.05),the anesthetic effects:group D was all excellent and better than the other groups (P<0.05); group B was better than groups A and C (P<0.05)7The analgesic effect in group D was better than the other groups (P<0.05)8Postoperative nausea and vomiting:group D was better than the other groups (P<0.05)Conclusionpropofol in combination with larger dose of dezocine (0.2mg/kg) can ge t efficient effect on sedation and analgesia,quick and perfect awakening,lowerre spiratory and circulation system depression,which is a better anesthesia for cho-ose. |
