| Purpose: Discriminate Synovitis in knee osteoarthritis by traditional Chinesemedicinebelong to TCM syndrome of hot and humid,carry out ComparativeEffectiveness Research in reducing swelling and relieving pain between treatmentof Relieve Swelling and Pain Prescription (constituted with rhizoma atractylodis,coix seed, cortex phellodendri, grifola, twotooth achyranthes root, rhizoma alismatis,Bi Xie, poria cocos, living by hemselves,rhizoma anemarrhenae, licorice root,notopterygium, salvia miltiorrhiza)which researched and developmented by TangZhenjiang who was tutor of doctor of traditional Chinese medicine in yunnanprovince,so as to provide objective clinical reason to chosen of treatment in curingSynovitis in knee osteoarthritis.Methods: Comparied the difference assessed by points which come from clinicalcurative effect evaluation of knee joint swelling, pain and function before and after2weeks between observation group and control group using the assessment method ofNIMO、VAS、WOMAC、Clinical Symptoms and Signs Scores,each group have30patients who distributed from60patients who have been selected。And follow-upthese2groups of patients in the next two weeks for observe the situation of their kneejoint functional response and relapse.Results: At last,the effective rate of observation group was90%with5patientshealed,17patients marked effect,5patients effectively,3patients no effect;of controlgroup was83.3%with3patients healed,13patients marked effect,9patientseffectively,5patients no effect.In score analysis，the WOMAC score=0.36F < F0.10(29,30), P>0.10, think the difference of two sets of homogeneity is equalbetween observation group and control group before treatment,so use independentsamples t test to compare, as the result t=0.387, P=0.707, P>0.05, so inference thedifferences between two groups is small, the two groups are comparable.theWOMAC score t=0.91, P=0.928, P>0.05, the score is not significant differencebetween bservation group and the control group, the information have no statisticallysignificanto after1week treatment,think clinical curative of drugs effect in the twogroups have on difference in the treatment of first week;After2weeks treatment,theWOMAC score t=2.628, P=0.011, P <0.05,the scores have significant difference,statistically significant, think clinical curative of drugs effect in the two groups havesignificant difference in the treatment of second week.The VAS pain score, checkedby F test, F=0.193<F0.10(29,30), P>0.10,think the difference of two sets ofhomogeneity is equal between observation group and control group beforetreatment,so use independent samples t test to compare, as the result t=0.233，P=0.871，P＞0.05，so inference the differences between two groups is small, the twogroups are comparable;The VAS score t=1.399，P=0.167，P＞0.05, the score is notsignificant difference between bservation group and the control group, theinformation have no statistically significanto after1week treatment;After2weekstreatment,the vas score t=2.941，P=0.005，P＜0.05,the scores have significantdifference, statistically significant;so think clinical curative in relieving knee painof drugs effect in the two groups have on difference in the treatment of first week butbservation group effect better than control group from the second week.In KneeSwelling score,checked by F test, F=1.162<F0.10(29,30), P>0.10,think thedifference of two sets of homogeneity is equal between observation group and controlgroup before treatment,so use independent samples t test to compare, as the result t=1.462, P=0.149, P>0.05, so inference the differences between two groups is small,the two groups are comparable;. The knee swelling score, t=2.604, P=0.012, P <0.05, the score is significant significant difference between bservation group and the control group, the information have statistically significanto after1weektreatment;After2weeks treatment,The knee swelling score t=4.811, P=0.0001, P <0.05,the scores have significant difference, statistically significant;so think clinicalcurative in relieving knee swelling of drugs effect in the two groups have significantdifference in the treatment of first and second and tbservation group effect better thancontrol group.The knee joint function WOMAC score, checked by the F test, F=0.426<F0.10(29,30), think P>0.10, think the difference of two sets ofhomogeneity is equal between observation group and control group beforetreatment,so use independent samples t test to compare, as the result, t=0.693, P=0.491, P>0.05, no significant differences of the two, is not statistically significant,two sets of scores are comparable. The knee joint function WOMAC score, t=0.018,P=0.986, P>0.05,the score is no significant difference between bservation groupand the control group, the information have no statistically after1weektreatment;After2weeks treatment,he knee joint function WOMAC score,t=0.12, P=0.905, P>0.05, the scores have no significant difference, statistically nosignificant;so think clinical curative inimproveing joint function of drugs effect inthe two groups have no significant difference in the treatment of first and secondweek, have no significant difference; Think in both medicine week1and week2, twogroups of treatment in improving all have certain curative effect on knee jointfunction, but their are no differences between two groups of medications.Afterfollowed up f，patients in observation group were5healed，15marked effect,5effectively,5no effect, total effective rate was83.3%，without change compared withthe2weekstreatment. In2weeks treatmentand2weeks followed up, its knee jointfunction WOMAC score t=0.781, P=0.485, P>0.05,have no significant difference;patients in the control group were were1healed，12marked effect,7effectively,10no effect, total effective rate was66.6%0%，with significantly change compared withthe2weekstreatment. In2weeks treatmentand2weeks followed up, its knee jointfunction WOMAC score,t=3.766, P=0.001, P <0.05,have significant difference.As the results showed,after2weeks.follow-up,observation group andcontrol group all can achieved clinical curative effectin in alleviating pain, relieveswelling and improve joint function, but observation group was superior to controlgroup in maintaining curative effect, improving knee function.Conclusion:Treatment of Relieve Swelling and Pain Prescription obtained goodclinical score, but from the comprehensive analysis, two kinds of therapies in thetreatment of knee osteoarthritis with synovitis all obtained good clinical curativeeffect; For relieving knee pain, two treatments had no significant difference after1week but significant after2weeks; For relieving knee swelling, whether after1weekor2weeks later, the differences effect of the two methods are significantly; For Kneefunctional score, the two groups of treatment have no significant differences, that twogroups of drug can obtained curative effect in terms of improving joint function, butthe difference is not big btween two groups. According to the results,followed up for2weeks,treatment of Relieve Swelling and Pain Prescription superior to "ibuprofensustained-release capsules in maintaining curative effect, improving knee function.Treatment of Relieve Swelling and Pain Prescription is a clinical curative effect torelieve pain and swelling of the knee joint to vast majority of patients with kneeosteoarthritis with synovitis, the drug curative effect more obvious after2weekstreatment and the effect keeps stably. |
