| The aging of the population is a problem faced by the social pension. As lack ofcommunity care service and pension institutions，it can’t meet the care and medicalneeds of the elderly. The pension institutions costs expensive，so home care becomethe main form of social pension. However, Most of elderly people have varyingdegrees of health problems, so they have high health needs. Some elderly can’t takecare of themselves，they need to be take care of. However，family caregivers lack ofreasonable care for the elderly，adverse effects often result in care giving，theyurgently need the professional guidance。Older people and their caregivers haveextremely high demand of health. Community health resources can’t meet theseneeds of the elderly, How to find a way to be able to take full advantage ofcommunity resources, to meet the needs of older persons and their caregivers, is aproblem to be solved.Research ObjectiveExplore a model of care to meet the needs of the elderly and their caregivers.The model can improve the care of the effect and reduce the care burden, andultimately improve the quality of home life of older persons.Research subjectsElderly people and their caregiversResearch MethodsIn this study, action research method was used. Includes the followingsections:①1The problem raised by the study that why there is such a high demand forhealth in elderly people and their caregivers was analyzed through the literatureresearch and Interview research.②2A nursing model of care was build throughtheoretical study and literature research.③3A revision was made for the constructedmodel by expert meeting④4A plan of the model was made and takenimplementation.⑤5The effect of the action was observed, and the results werereflected.Research results1.The problem-solving approach was obtained through the analysis of theproblem：①1elderly health problems is the root causes of high demand；②2the elderlyand their caregivers should be a unified object of study；③3interactive nursing couldbe a method. 2. Based on the idea of Orem’s self-care theory, the king’s theory, thenurse-patient relationship theory,family systems theory,the client health behaviorinteractive mode, interactive process theory and Omaha home visits, the elderly-caregivers-nurses interactive care model was constructed.3. The assessment of the health of the elderly at home and the outcome of theevaluation index was selected.4. A Revision was made for the nursing model and assessment tool.5. The effectiveness:60pairs in yangpu of Shanghai district participated in this study voluntarily.Main research results are:(1) fully participation rate was98%;(2)93%of the elderly-caregivers can meet the target according to the plan;(3)85%of the elderly-caregivers said it can solve the health problems in life;(4) the medication complianceof elderly increased by27%;(5)83%of the elderly and their caregivers weresatisfied with the nursing mode.ConclusionIn this model，the elderly and their caregivers as a whole nursing objectinvolved in the care work together and maintain a good interaction with the nursesin the nursing activities, it is a good way to solve the current problems. This caremodel can maximize the self-care ability of the elderly and improve the care abilityof the caregivers, enhance care of efficacy; it also be able to application the familyresources reasonable and reduce the use of health care resources. It is a usefulsupplement for the current home in retirement, alleviate aging caused by thecontradiction between supply and demand of health care services. There is a greatsignificance to ease the contradiction between supply and demand of health careservices caused by the aging. |
