Retrieval And Analysis Of Three Kinds Of Important Diseases In Department Of Ophthalmology Of Evidence-based Medicine Literature

Evidence Based Medicine is one of the hot spot of research by Medical Library andInformation science specialists and clinical workers. Evidence-based medicine is thetheory and method of evidence-based medicine in ophthalmology application becomeevidence-based ophthalmology and provide the basis for clinical diagnosis and treatment.The purpose: According to the clinical and the existing literature. We have retrievedliterature of the general eye disease. this study is based on theory and select the three kindsof eye disease.(Ocular trauma Anterior ischemic optic neuropathy and Gravesophthalmopathy). It guided by the EBO theory and principles which retrieved andanalysed of the eye diseases and Meta-analysis. To determine whether the retrievedliterature analysis results formed has higher guidance meaning for clinical practice ofprimary research evidence.In standard textbooks and scientific works, It is divided into5levels following theevidence in the medical literature research. The object of this study is the one level ofevidence.The homogeneity of randomized controlled trials of systematic review(Oxford levels of evidence in2011and Strength of recommendation), some monographs will beinterpreted as level1evidence:①Systematic review and Meta-analysis.②Thestandard and grasp a high degree experimental study(RCT).The method: The medical scientific retrieval method,Bibliometric method andMeta-analysis and so on.The result: On the basis of literature retrieval of Corneal disease, focusing on the literatureof therapeutic aspects, to review the literature for clues, Literature aiming to realizeMeta-analysis (with the effect of the amount of data) as the standard. Filter out thefollowing three types of disease(Ocular trauma Anterior ischemic optic neuropathy andGraves ophthalmopathy) as case study, carrying out Meta-analysis among the extensiveliterature. Screening of these three kind of disease is the cause of literature appeared morehomogeneity randomised controlled trials of systematic review for Meta analysis.For scientific Meta-analysis, searchable database range in the literature and other sourcesof literature has been expanded, the latest literature, but also a small amount of the earlierliterature were selected.On the platform of database as CBMdisc, Cochrane Library,PubMed, Embase and so on.We made a comprehensive literature search, as a result, theamout of Ischemic optic neuropathy literature was1575(Chinese1003, English414),which of Ocular trauma disease was1953(Chinese218, English1200), and Gravesdisease1080(Chinese280, English420). Among all the Ophthalmology desease literature,which a few can meet the Meta-analyse standard:32for Ischemic optic neuropathy,11forOcular trauma disease and6for Graves disease.According to the results,we can reach conclusion.(1) Ocular trauma Q﹦12.58，P<0.1，theresearch data is homogeneous. Combined effect quantity inspection reached95%confidence interval4.77[3.20,7.11]，Z=7.68，P<0.0001，It has statistical significance. Theforest figure reached the above test results intuitively. The funnel chart reached publishedsex bias is not obvious.(2) Anterior ischemic optic neuropathy Q=13.9，P<0.1，theresearch data is homogeneous.95%confidence interval5.71[4.38,7.43]，Z=12.93，P<0.0001，It has statistical significance. The forest figure reached the above test resultsintuitively. The funnel chart reached published sex bias is not obvious.(3) Gravesophthalmopathy Q=1.09，P>0.1，the heterogeneity is larger(Heterogeneity test efficiencyis low).95%confidence interval5.71[0.60-1.64]，Z=-0.03，P>0.01，It has not statisticalsignificance. The funnel chart reached published sex bias is obvious.We can see that Meta-analysis as the core method of ophthalmology evidence-basedmedicine, provides more systematic and scientific information for a level1evidence ofclinical diagnosis and treatment. But at the same time it should be noted Meta-analysis asa secondary analysis, existing confounding bias, should be correct understanding andrational use of the results of the Meta-analysis.Ocular trauma and Anterior ischemic optic neuropathy Literature aiming to realizeMeta-analysis formed the EBO level1evidence, It has more higher statistical significancefor clinic.
