Background In China, we have not achieved separation of drug prescribing and dispensing and this brings high medical cost and the corruption. In order to control and alleviate the situation, the government had enacted a new medical reform policy in April6,2009, which clearly declared to propel separation of drug prescribing and dispensing and explore a variety of effective ways to reform to medical system. Recent years, the separation of drug prescribing and dispensing has become a hot topic.Objectives The thesis is aim to use the Transaction Cost Theory in drug prescribing and dispensing, exploring effective measures to promote the separation of medicine based on the Transaction Cost Theory.Contents The research contents in the thesis:(1) The application of Transaction Cost Theory in separation of drug prescribing and dispensing;(2)Surveyed the patients’buying drug behavior, studied the factors of outpatients selected the pharmacy;(3) Compared the drug price between hospital pharmacy and the retail pharmacy and researched the health policy, from multiple views studied the factors of outpatients selected the pharmacy;(4)To explore effective measures to promote the separation of medicine based on the Transaction Cost Theory by analyzing survey data.Methods The methods include Document Research, Questionnaire, Interview, T-test, x2Test and other statistical methods. The Document Research was mainly used to research of the Transaction Cost Theory and separation of drug prescribing and dispensing in the aspect of theory, including review and summarize the origin, development, features and applications of Transaction Cost Theory. What’s more, the Document Research also used to design the questionnaire of outpatient’buying drug and health insurance research. Questionnaire mainly for the purchase of medicines outpatient case investigated, and Interviews primarily for health insurance office staff and outpatients" research. Describe and analyze survey results by using T-test, x2test and other statistical methods.Result In the survey on outpatients there were97.08%of respondents selected the purchase of medicines in hospital pharmacies after the last visit of doctor, prescription highly concentrated in the hospital pharmacy. In addition,73.33%of respondents prefer drugs from hospital phannacy rather than retail pharmacy.43.54%of respondents believe the drug quality of hospital is better than that of retail pharmacy.55.83%believe the drug from retail pharmacy is cheaper. On the Interviews found that different regions had different Health Insurance Pharmacy policy. Some areas can directly use the Health insurance card to buy drugs, while other areas must hold an invoice back to health insurance reimbursement. There were no difference between Beijing respondents and other places respondents about knowing the Health Insurance Pharmacy. But respondents who lived in Beijing less buy drugs than other district respondents in Health Insurance Pharmacy. Compare prices on drugs found in the same factory specifications same brand same medicines, hospital phannacies and retail pharmacy price is basically the same.Conclusion Both from the economic view of Transaction Cost Theory or from the interests of the medicine chain analysis,promoting the free flow of prescription medicine can help to achieve separation of drug prescribing and dispensing.However, the survey found that prescription is highly concentrated in the hospital, the impact of factors including not only hospitals restrictions on prescriptions, as well as health insurance policy, the patients consumption habits, drug prices, drug quality and many other effects. Health insurance policy, control measures from hospital and drug manufacturers as well as consciousness and purchasing habits of outpatients are factors which restraining prescriptions outflow from the hospital.
