OBJECTIVE:To provide a guideline for the establishment of the charging standard of dispensing service, we measured the total cost and project cost of dispensing service provided by medical institution, explored the dispensing cost of outpatient pharmacies and emergency pharmacies of3A grade hospitals in Changsha and analyzing the dispensing cost; To provide the guideline for the further research, we analysed the principal factors affecting the dispensing cost and exploring a reasonable research scheme.METHODS:A questionnaire was designed to collect the information. Professional staffs from department of pharmacy are in charge of filling in the questionnaire. After the data collection, specific accounting method was used to analyze the total cost and unit cost of dispensing service in the outpatient pharmacies and emergency pharmacies from different hospitals. Make the data statistics and analysis was made of the sensitivity analysis was deduced to test the related factors affecting the cost of dispensing service.RESULTS:(1) In2010, the total service cost of outpatient pharmacies and emergency pharmacies was8.18million Yuan in general-level3A grade hospitals,1.86million Yuan in provincial-level3A grade hospitals,0.96million Yuan in provincial-level maternal and child special hospitals,0.62million Yuan in provincial-level tumor hospitals,1.77million Yuan in provincial children special hospital,0.59million Yuan in provincial hospital of traditional Chinese medicine, and0.4million Yuan in prefecture-level3A grade hospitals. And the human resources cost was huge in all the hospitals.(2) The average per patient cost of dispensing service from outpatient pharmacy and emergency pharmacy was about1.90Yuan,2.75Yuan per one prescription, and0.89Yuan per one drug entry. The unit costs of dispensing service of hospitals per person-time of patients are mostly smaller than per prescription. Calculating the unit time and unit cost of dispensing service of outpatient pharmacy and emergency pharmacy on the basis of the drug entry in prescriptions, unit time of dispensing the dilute-needed medicine, dismounted medicine and special control drugs is longer than dispensing the popular medicine and the unit cost of them is also higher than others.(3) The human resources cost change has much effect on unit cost, and equipment cost change has bigger impact on unit cost in Xiangya hospital than in other hospitals.CONCLUSIONS:(1) To compare the two methods (hospitalization number and prescription number) in calculating the unit cost, it’s more reasonable to establish the dispensing service charge standard on the basis of the drug entry in prescriptions.(2) Calculating the service cost by per drug entry, the dispensing service cost is largely depended on average dispensing time of drug.(3) Human resources cost has much effect on the total dispensing service cost, and the application of automated high-value equipments can enhance the influence of device cost on total dispensing service cost, which should be taken into account in the establishment of reasonable charge standard.
