ObjectiveBeing the essence of China, Traditional Chinese medicine (TCM) is part of world medicine. TCM experts are special treasures of this medicine for they have developed academic ideas as well as their clinical experience through decades of repeated medical practice. What they have achieved lead TCM academic and clinic advance. Their achievements are so brilliant that it is important to collect and study their academic ideas and clinical experience for they are the worthy knowledge. Also the collection and study is an approach to inherit and improve our traditional medicine. To inherit what our experts have achieved is what we need to do now, in this way we are going to collect and study their academic ideas and clinical experience to help the improvement and promotion of our medicine. That’s what we have done counts.Guan Baolian, Chief Physician, from Man national, born on09-09-1964in Shenyang. Graduated from TCM Faculty of Inner Mongolia Medical College in1964, she later accomplished her medical advanced courses for3years in Shanxi College of TCM since1976. She got a good reputation in the field of cardiovascular and respiratory system and is very good at syndrome differentiation and treatment with quite abundant clinical experience. Her knowledge of TCM is not only profound but also integrated with modern medicine. She has something different to various diseases especially to the heart and lung diseases and her treatments are always proved to be effective. She has leading some research teams to study "coronary diseases","high blood pressure" and "hyperlipidemia" since1983. Four academic programs including "Clinical and Experimental Study on Herbal Series for Coronary Disease" were awarded the Fourth Prize. She has more than20papers published. In shorts, it is really necessary to study her academic ideas and clinical experience.Methods1. During the three year apprenticeship study, we mainly follow her treating style during outpatients’ visits. We copied and studied her prescriptions for different cases and we concluded what we have learned monthly and regularly.2. Learn the classical works such as Neijing (Internal Medicine), Shanghan Lun (Treatise on Cold Damage Disease), Jingui Yaolve (Synopsis of Golden Chamber) and Wenbing Tiaobian (Differentiation of Febrile Diseases) lectured by our teacher and we taught to focus those parts concerning the heart and lungsystem.3. We are equipped with professional treating methods and syndrome differentiation techniques of historical professional for the heart and lung diseases. 4. Guide the clinical practice and summarize timely what we have learned. Results1. What is our teacher’s academic idea and how she gets itShe concentrates on the relation between the lung and other organs while treating the heart and lung diseases.2.1The Lung1) Govern the qi transformation:She explains the function of the lung from qi transformation, pathological manifestations and treatment of sanjiao.2) Control Zongqi:She explains the lung controlling zongqi from production and transportation of zongqi, related recordings in Neijing and her understanding of those recordings.3) Govern Yingqi and Weiqi:She explains the function of the lung yingqi and weiqi from the production and transportation of them as well as related recordings in Neijing and her understanding of TCM diaphoresis.4)Govern the Ascending Yangqi:She explains the lung governing ascending yangqi from the production and related recordings in Neijing and her understanding of TCM cause of asthma.In all, the abnormal function of the lung might have adverse impact on the lung system and other systems, especially on the heart system.2.2Relation between Organs1) The heart and lung are supplement each other, so do qi and the blood.2) The heart and spleen promote each other, so blood can flow smoothly.3) The lung and spleen promote each other, so do fluid and qi.4) The lung and kidney promote each other, so do yinqi and water.5) The heart and kidney communicate with each other, so do the water and fire.6) The spleen and kidney help and promote each other.7) The liver and kidney share the same origin, so the essence and blood can transform to the other.8) The spleen and stomach help each other and control the ascending and descending of qi mutually.We have illustrated the relation between organs from the aspect of physiology and pathology. A Summary of Our Teacher’s Academic Ideas and Clinical Practice3.1Introduction to Her Academic Ideas1) Emphasize Holistic Concept and Organ RelationsWe have summarized her understanding to the relationship of Twelve Organs in Linglan Midian Lun of Suwen (Chapter of Discussion on Imperial Library and Secret Collections from Plain Conversations) and we now believe that the human body is a whole and organs are closely related to each other. If one of the organs is attacked, the others are likely involved, or even both zang and fu organs are both involved. Such an understanding is quite fit for coronary disease, chronic pulmonary heart disease, various refractory heart failure, related therapeutic idea are proved to be effective such as treating the heart, the stomach and gallbladder at the same time, treating the heart and lung together, treating the heart and kidney together.2) Focus on Yin and Yang Balance with Reasonable PrescriptionsShe pays more attention to the balance of yin and yang and always stresses the importance of restore the balance of them from diagnosis, treatment to prescription. We concluded her ideas and experience.3) Reinforce the Healthy Qi while Trying to Remove the Pathogenic Qi, Focus on the Spleen and Stomach, Basis after BirthWe have summarized her understanding of the importance of the spleen and stomach theoretically and practically.4) Pay Attention to Lab Findings-Supporting Evidence of Syndrome Differentiation and Treatment.We concluded her techniques of apply lab findings to clinical practice, those findings are another supporting evidence for TCM syndrome differentiation and treatment.3.2Introduction to Her Clinical Practice1) AsthmaWe concluded that her treatment for asthma is "3step9methods". That is her treatments for asthma are different because causes and pathogenesis change according to the different stages such as acute stage, chronic stage and recovering stage. 2) CoughWe concluded her four methods on cough as "descending, regulating, communicating, transforming" and often used herbs.3) Diffuse Interstitial Lung DiseaseWe summarized her experience on it from diagnosis, syndrome differentiation, therapeutic principles, prescriptions and typical cases as well.4) High Blood PressureWe concluded causes, pathogenesis, diagnosis, syndrome differentiation and treatment for HBP.5) Coronary DiseaseWe illustrated causes and pathogenesis and therapeutic principles for it and how her own patent herbal series "Guanxin Shu (Coronary Disease Relief)" work. We have also concluded syndrome types and typical cases of coronary disease.6) ArrhythmiaWe have concluded syndrome types of it.7) HyperlipidemiaWe have concluded syndrome types of it.ConclusionDuring the three year apprenticeship study, we mainly follow her treating style and we concluded her academic ideas and her prescription style, clinical experience on heart and lung diseases and we hence have improved a lot. Objective:To analyze the pathogenesis of chronic persistent asthma and propose the treatment of reinforcing deficiency, dispelling wind and expelling phlegm. By observing the clinical curative effect of jiao long ding chuan tang in treating chronic persistent asthma and to explore its possible mechanism.Method:By analyzing Literature reviews and summaries of research from the field of Chinese and Western medicine on pathogenesis of asthma, reports on the treatment of the disease in recent years, reviews on the development and research status of treatment of asthma and characteristics of etiology and pathogenesis of chronic persistent asthma, propose the treatment of reinforcing deficiency, expelling wind and expelling phlegm, using jiao long ding chuan tang in treating the disease, providing theoretical basis for research. By observing clinical symptoms and ACT scores, Serum IgE levels before and after treatment, the peripheral blood eosinophils (EOS) count and lung function (FEV1, and PEF), the evaluation the effect of both observation group (group of jiao long ding chuan tang) and control group (Group of Seretide)can be measured in the treatment of chronic asthma.Result:There is significant statistical differences ((P<0.05) in the total effective rate of two groups, The significant efficiency of the test group is much better than the control group (p<0.05). Both groups showed good results in improving main symptoms such as cough, gasp, chest tightness, wheezing, intolerance of wind, and polypnea. Experimental group has better results than the control group (p<O.05)in expectoration, chest tightness, wheezing, gasp, intolerance of wind and polypnea. There is no significant statistical differences (P=0.117) in cough. In terms of improving ACT scores, significant increase in ACT scores after treatment in both the two groups was found, with the experimental group better than the control group (p=0.021).Peripheral blood eosinophils has both dropped after the treatment in two groups, with no obvious statistical significant difference (P=0.101). Lung function,as to FEV1, has increased after treatment in both of two groups, but no obvious statistical difference (P=0.54) PEF increased significantly, and there is statistical difference in two groups after treatment (P<0.05).Conclusion:Clinical research have shown that using reinforcing deficiency, dispelling wind and expelling phlegm on chronic persistent asthma, has better therapeutic effect in improving the symptoms and signs of patients. It also has certain advantages in improving ACT scores, IgE and PEF. The mechanism may be related to its antiallergiceffects, elimination of airway inflammation, reduces airway hyperresponsiveness, improving the potential airway obstruction and other effects during the chronic persistent period. |
