Objective:this project consists of the animal experiment and clinical observation two parts.The clinical observation part is to observe the clinical curative effect of Bushenqudu Decoction through collecting cases.The animal experiment part is to observe the influences of Bushenqudu Decoction on kidney function,renal tissue form and TGF-β1/Smads signal transduction pathway of chronic renal failure rats which are induced by adenine through the experimental study,and discuss the mechanism of Bushenqudu Decoction on delaying the progression of chronic renal failure and against renal interstitial fibrosis.Methods:clinical part:collect60outpatients and inpatients with chronic renal failure of nephrology department of the hospital from2012January to2013January,then divide randomly into treatment group (30cases) and contrast group (30cases) according to treatment sequence.Treatment group is treated with routine western medicine and Bushenqudu Decoction,control group is treated with routine western medicine and Niao Du Qing Ke Li.Observe one course of treatment which is two months.Contrast the change of serum creatinine (SCr),blood urea nitrogen (BUN),creatinine clearance rate (Ccr) and urineTGF-β1between before treatment and two months after treatment.Experimental study part:76rats are randomly divided into6groups:normal group, model group,benazepril group,NiaoDuQing group,Bushenqudu Decoction Squama Manitis group and Bushenqudu Decoction Carapax et Plastrum Testudinis.Make rat renal failure model with adenine gastric lavage.After30days of giving the corresponding medicine gastric lavage treatment,test the SCr,BUN, urineTGF-β1and TGF-β1,Smad2,Smad3,Smad6by immunohistochemical method and TGF-β1mRNA by real-time fluorescence quantitative PCR and Smad2,Smad3,Smad6by western blot of every group rats,and observe glomerulus and renal tubule interstitial lesions of every group rats by pathological morphology.Results:Bushenqudu Decoction can obviously meliorate the renal function of patients,delay renal failure process,reduce the BUN,SCr, urineTGF-β1level of the chronic renal failure rat, and there is a significant difference compared with the contrastl group;can increase hemoglobin level,but no significant difference compared with the contrast group;can reduce the expression of TGF-β1and Smad2/3protein in renal tissue of chronic renal failure rats and increase the expression of Smad6protein,and there is a significant difference compared with the contrast group;can cut down the expression of TGF-β1mRNA in renal tissues,and there is a significant difference compared with the contrast group.Conclusion:Bushenqudu Decoction may inhibite the signal transduction pathway which TGF-β1leads to the nucleus through increasing the expression of Smad6protein in renal tissue of chronic renal failure rats,reducing the expression of Smad2/3and TGF-β1protein. Bushenqudu Decoction can reduce the renal interstitial fibrosis,delay the progression of chronic renal failure by affecting the transduction of TGF-β1/Smads signal pathway. |
