Objective:In order to explore the Pharmacodynamic mechanism of Bushenqudu decoc-tion and provide a basis of modern relevant research and practical basis for its Clinical application.Interfere the treatment of rats which suffered from the adenine-induced chronic renal failure, through the clinical and experiental research went on simutaneously,combining the modern research method.Methods:(a):From March 2010 to january 2011,64 cases inpatients and outpatients with chronic renal failure were chosen by adoption standards and exclusive criteria in the Second Affiliated Hos-pital of fujian University of TCM,were divided randomly into two groups,whi-ch including 32 cases treated with on the basis of conventional western medicine plus bushenqudu soup as treatment group and 32 cases treated with conventional western medicine plus niaoduqing Granule as control group,observing for one period of treatment and 2 months made up of a course.The clinical symptom score,SCr,BUN,Ccr,Hb,ALB and CRP were comp-ared between pretreatment and 2 months after treatment in the constipation patients. (b) Expe-rimental research:80 rats were randomly divided into 8 groups,they were normal group, model group,bushenqudu groups of high dose,medium dose and low dose,niaoduqing groups of high dose,medium dose and low dose,experimental model of adenine-induced chronic renal failure rats were feeding medicine respectively for 35 days after which SCr,BUN,CRP,IL-6,IL-10 and TNF-a were detected,and the change of the ultrastructure of kidneys were observed in each group.Results:(a) the clinical research showed that the total effective rate of the experimental group was 93.75%, better than that of the control group which was 71.87%.The SCr,BUN, Ccr and CRP tests, and clinical symptoms of the experimental group were greatly improved compared with that of the control group. But there was no significant difference between two groups in Hb and ALB.(b)Experiments researches had shown that experiment groups were decreased significantly compared with the model group in SCr,BUN(P< 0.05); bushenqudu groups of high dose and low dose was less than that of the niaoduqing groups, the difference was very significant(P<0.01).but there was no difference between that of the medium dose(P>0.05). and The treatment group and serum CRP,IL-6, and TNF-a content was less than that of the niaoduqing groups (IL-10 was more than),the differences were statistically significant(P<0.05or P<0.01).The pathological damage in bushenqudu and niaoduqing groups was lower and better than that of in model group, bushenqudu groups are the best.Conclusion:bu shen qu du decoction can improve patients'clinical symptoms and signs obviously,lower the Scr BUN,CRP of patients effectively and improve Ccr.Moreover,it can help reduce the pathological injury of rats'kindey and reduce inflummatory cell in kidney,thus delay the deterioration of kindey.It follows that the bu shen qu du soup is an effective prescription in curing chronic renal failure,it deserves to popularize and Its mechanism can be the restraint or reduce mesangial hyperplasia,the permeability of endothelial cells, the lumenal sclerosis and so on, which were caused by CRP; Its can be inhibit or reduce the level of IL-6 expressions,which can influence on renal interstitial fibroblasts,because of the unbalance of synthesis and degradation of the accumulation of extracellular matrix;And inhibit or reduce injured renal tubules and cytokines network caused by TNF-a.And Promote the level of IL-10 to restain toxic of inflammatory cytokines.Thereby preventing the development of excessive inflammatory response, and delay the loss of renal function.
