ObjectiveThe thesis aims at forming supply and demand balance theory to offer guidance for nursing staff and hospital administrators in order to improve their services, enrich the concept of good quality nursing service and enhance patient satisfaction through exploring high quality nursing service work. The study reaches a qualitative theory model, describes the concept of each component and their relationships, and provides certain theoretical basis for the development of late scale. This study applies qualitative research in nursing. The theoretical model in strict accordance with the sociology paradigm provides a sample for future qualitative research in the nursing field.MethodsThis study was a qualitative study uses convenience sampling method, takes7hospitals,20nursing personnel55patients in Jinan research objects. The time range was April2011to March2012, coincides with the period of national high quality nursing service movement. The research analyzes information acquired by interviews and observations among nursing personal and patients adopting the open, axis and core principles of grounded theory3-level coding method, code forming and cross reference, members inspection, interview to improve reliability. The paper finds the correlations between each element and constructs the balance theory by dividing the objectives into five elements, namely patient needs, services provided by nursing staff, patient support systems, nursing personnel support systems, supporting points of high quality nursing service.At the same time, the thesis using SPSS descriptive statistics to analyze research materials, the basic information of hospitalized patients counting materials, and uses χ2inspection to compare different statistical differences of count data. The whole research process is divided into two stages as follows.1. First stage phase one, from April2011to August2011, site observation of the "demonstration ward" at provincial hospital in Jinan, Shan dong. Interview11nursing personnel,10patients, form the initial high quality nursing service supply and demand balance grounded theory. First stage phase two, from September2011to November2011, interview9nursing staff from eight departments of other hospitals in Jinan. Review the transferability of proposed theory and divide the involvement departments according to the theory.2. Second stage, from December2011to March2012in March, apply the balance theory to TB wards at a provincial hospital. Observe and interview15hospitalized familial TB patients in, construct" Family Infection Evaluation Form of Infant TB". Discover patient needs and provide targeted professional nursing and health education to patients through evaluating and analyzing the knowledge and protective behavior of particular patient group.ResultsBased on leverage principle (i.e., weight X weight moment=commodity X commodity moment), put forward "high quality nursing service balance theory. Treat nursing services as weight, patient needs of nursing service as commodity, nursing support system as weight moment, patient support system as commodity moment and national reform policy the balance the supporting point. Divide the8hospital departments mentioned in phase two of the first stage into three types:leading mature type (n=2), developing type (n=5) and acceptance type (n=1), respectively name them Class One, Class Two and Class Three departments, test and enrich the balance theory further. In Stage two clinical nursing, the nursing support system, patient support system, support factors are relatively stable. Satisfy patient demands using a variety of methods through analyzing patient needs from building the "Family Infection Evaluation Form of Infant TB", and as a result, test the feasibility of the balance theory.ConclusionThis research strictly follow scontent analysis steps of qualitative research. The "High quality nursing service balance theory" it reaches is original and scientific, and offers guidance to the clinical nursing. The operation model of class one departments shows that balance and development is the core of quality nursing. The existence of class two and three departments under the same social condition reflects the "balanced sustainability" contradictions facing quality nursing service, and the needs of further explorations in reforming nursing management model, building scientific performance evaluation system and actively adopting information system etc. the application of balance theory in TB wards indicates that using the balance theory in quality nursing as guidance can guarantee the complementation of basic nursing service, lift up nursing level,’ promote the development of specialized nursing service, demonstrate the meaning of professional nursing, prompt nursing personnel to form noble values and dedication, and constantly improve the advancement and scientific nature of the nursing discipline. |
