Glaucoma is a serious irreversible blind eye disease, Trabeculectomy,as one of the most common clinical glaucoma surgery, depends on the different kinds of organization healing in operation area, making the eye aqueous outflow to the cornea.Filtering bleb scarring blocke the channel aqueous channel to the obstruction, increased intraocular pressure, is the main reason for surgical failures.At present the most common clinical resistance to scar medicine is MMC,etc. which has improved effective, but with side effects. It IS necessary to search for effective and safe method to restrain filtering bleb fibrosis. Vascular endothelial growth factor (VEGF) is the most important factor in the process of angiogenesis,TGF-P has been proved to be a key function factors in the process of the wound repair,our study is to observe the expression of VEGF and TGF-β2of the rabbit eyes after trabeculectomy in filtering bleb and prove that VEGF and TGF-P2can be inhibited by avastin.The first part:Dynamic expression of vascular endothelial growth factor and transforming growth factorβ2in Filtering bleb of rabbitObjective To observe the Dynamic expression of vascular endothelial growth factor (VEGF)and transforming growth factorβ2(TGF-β2) in Filtering bleb of rabbit,and to determine the relationship between VEGF and TGF-P2. Methods Trabeculectomy was performed in one eye of28chinchilla rabbits,with another eye as a blank control randomly,in1,3,5,7,14,21days after operation inflammatory and blood vessel reaction of filtering bleb was observed, the VEGF and TGF-β2was measured by ELISA. Results Compared with the control group, the VEGF and TGF-β2obviously increased and reached to paek in fifth and seventh day respectively (P<0.05).the VEGF and TGF-β2had correlation (P<0.05).Conclusion The VEGF and TGF-β2of filtering bleb was closely related to filtering bleb scarring and correlative to each other.The second part:Inhibiting effect of avastin against vascular endothelial growth factor and transforming growth factorβ2in Filtering bleb of rabbitObjective To prove the ihibiting effect of avastin against fltering bleb scarring and vascular endothelial growth factor (VEGF),nsforming growth factorβ2(TGF-β2) in Filtering bleb of rabbit.Methods20rabbits were randomly divided into four groups:A, B, C(experimental group)and D (blank control group) each group of10eye, all performed tabeculectomy. Rabbits of A group and B group were injected under conjunctival by1.25mg and2.5mg Avastin respectively,those of C group were posted by cotton piece with0.25mg/ml MMC under conjunctival and sclera disc, those of D group were injected balance salt solution under conjunctival.Filtering bleb form and function, adverse reactions and complications were observed in3,7days after operation,the number of fiber cells were compared by light microscopy and the VEGF and TGF-β2was measured by ELISA in7days after operation. Results Fiber cells of B group were less than those Aã€Band D group significantly (P<0.05),there was no significant statistical difference in the number of fiber cells in A and C group. VEGF of filtering bleb in each two groups were different significantly,from Less To More was Bã€Aã€Cã€D group. TGF-β2of filtering bleb in Bã€C groups was less than Aã€ã€D group, there was no significant statistical difference in the number of fiber cells in B and C groups. Conclusion VEGF and TGF-beta2was closely related in filtering bleb scarring and inhibited by Aastin,which can restrain filtering bleb scarring after trabeculectomy sfely and effectively. |
