Objective:To evaluate the insulin resistance and investigate the serum concentrations of RBP-4and IL-6related to insulin resistance in patients with Graves disease and type2diabetes.Methods:One hundred and seven patients with Graves diease were divided into initial diagnosis group(n=54) and treatment group (n=53). Type2diabetes group(n=53).Fifty healthy subjects were selected for the control. We collected the clinical date include name,gender,age,weight,height and so on. The levels of glucose, insulin, free triiodothyronine, free thyroxine, thyrotropin, antithyroglobulin antibody, anti-thyroperoxidase antibody and anti-thyrotropin receptor were detected using fasting blood. The serum levels of RBP-4and IL-6were detected by enzyme-linked immunosorbent assay (ELISA).Result:1.There were no significant difference between each groups in gender,age and BMI. The fasting blood-glucose level in type2diabetes group was higher than other groups(P<0.001,P<0.001,P<0.001).There were no significant difference between other groups.The fasting insulin level of type2diabetes group was the highest in four groups and the control group was the lowwest.Initial diagnosis group and treatment group were in the middle,and there were no significant difference between these two groups(P>0.05). In initial diagnosis group,there were no correlation between FBG,FINS,HOMA-IR and FT3,FT4,TSH (P>0.05)2. The seurm level of RBP-4in control group, initial diagnosis group,treatment group and type2diabetes group increased in turn(P<0.001). There were no significant difference between treatment group and type2diabetes group(P>0.05).The seurm level of IL-6in initial diagnosis group was higher than other groups(P<0.001). There were no significant difference between other groups(P>0.05). In initial diagnosis group,there were no correlation between RBP-4and FT3,FT4,TSH(P>0.05).The serum level of IL-6was positive correlation with FT3and FT4(r=0.493, r=0.650).There were no correlation between IL-6and TSH(P>0.05)Conclusion:1. Graves disease patients did not exist explicit glucose metabolism disorders. Patients with type2diabetes existed obvious insulin resistance. The patients of initial diagnosis group and treatment group also existed insulin rsistance,but the degree of insulin resistance was less than type2diabetes group.The correlation analysis suggested that there were no correlation between the degree of insulin resistance and thyroid hormone level.2.The serum level of RBP-4was increased in Graves disease and type2diabetes patients. The serum level of RBP-4in initial diagnosis group was lowwer than treatment group.It was suggested that thyroid hormone level and immunologic system affected the serum level of RBP-4. The serum level of IL-6could be as Graves disease course observation index. The correlation analysis suggested that there were no correlation between the serum level of RBP-4and thyroid hormone level.Tyroid hormone may not affect the serum level of RBP-4.But tyroid hormone could decrease the serum level of IL-6.3.Insulin resistance in Graves disease patient may relate to high serum level of RBP-4or IL-6.
