Background and ObjectiveCardiovascular diseases (CVD) are the leading cause of death in patients with end-stage renal disease (ESRD). To compare with the general population, there are not only common risk factors, as dyslipidemia, hypertension and diabetes, in such patients, but also microinflammatory state, endocrine and metabolic disturbance, oxidative stress, anemia, endothelial cell disfunction. Because of these, there is a high morbidty of CVD, and may be a different pathogenesis of CVD in ESRD. Study of matrix metalloproteinase-10(MMP-10) show that MMP-10has a important role in the progresses of tumor and pelvic organ prolapse (POP), and an association with atherosclerosis. The aim of this study is to determine whether there is an association between matrix metalloproteinase-10and atherosclerosis in patients with ESRD and then to explore the role of MMP-10in the progress of atherosclerosis.Materials and Method:All individuals were sampled randomly from our hospital,60patients from the center of blood purification,30healthy subjects from medical examination center whose carotid intima media thickness (cIMT) are normal. The subjects of hemodialysis patients were selected according to the followed criteria:a) hemodialysis therapy regularly not less than3months; and b) age between35and65; c) no history of malignancy, autoimmune disease, severe infection, acute stage of infection, congenital heart disease, valvular disease, severe arrhythmia, depletion, an expected death in6months, severe aanemia (HGB<50g/l), haematological systemic disorder. All blood samples were taken in the morning without any food. Glucose, lipidemia (cholesterol total, triglyceride, high density lipoprotein cholesterol, low density lipoprotein cholesterol, very low density lipoprotein cholesterol), renal function, albumin. Routine blood test, calcium, phosphorus, high sensitive C-reactive protein are tested in blood samples of HD subjects but not in healthy subjects. Serum are segregated in the centrifuge. MMP-10and TIMP-1are tested with Elisa test. The subjects’characteristics are recorded and the cIMT is tested by a professional physician. SPSS17.0software application was used for all analysis and statistical significance was defined as P values less than0.05.Results:1)There is significant difference of systolic pressure, diastolic pressure, Glu, Alb, BUN, Cr, HDLc, MMP-10, TIMP-1, IMT between two groups.2) There is positive association between serum MMP-10and age, hsCRP, IMT, PROCAM, but a negative association between serum MMP-10and Alb (P<0.05). We don’t find significant correlation between MMP-10and TMP-1. cIMT is positively and significantly associated with age, PROCAM, MMP-10(P<0.05). We don’t find any significant correlation between TIMP-1and other items.3) The patients with increased cIMT (χ2=8.938, P=0.030) and plague (χ2=8.782, P=0.032) are increased with a higher concentration.4) Logistic stepwise regression analysis reveals the increased LDL-c/HDL-c may be a risk factor of the increased cIMT in patients with ESRD. Overweight and increased cIMT may be risk factors of the plague(P<0.05) in patients with ESRD.Conclusion:MMP-10is positively and significantly associated with cIMT and plague, but MMP-10is not the risk factor of them. |
