Objectives:1. To understand patients with post-stroke depression in social welfare institutions;2. To study the rehabilitation intervention programs of patients with depression after stroke through an evidence-based care approach;3. To assess the impact of intervention programs for post-stroke depression in patients on depression, quality of life.Methods:1. In this study stroke patients living in the community welfare centers in Wuhan City for the investigation of secondary screening using the Hamilton Rating Scale for Depression (HRSD) and the Mini-Mental State Examination (MMSE) after stroke depression and no recognized known dysfunction of patients with60cases, using random number table method of post-stroke depression patients were randomly divided into intervention and control groups, each30cases.2. Into the study, issued a general demographic questionnaire, the Mini Mental State Examination (MMSE) and Hamilton Depression Rating Scale (HRSD) and determine the information of the general population of patients, severity of depression and cognitive status; again payment of stroke specialized quality of Life Scale (SS-QOL), from the three dimensions of physical, psychological and social evaluation of the depressive state, to understand the current situation of the PSD in patients with depression, a clear evidence-based nursing problems.3. Access to domestic and foreign literature to find the empirical evidence-based problem, and prudent, rigorous evaluation of the empirical literature, to filter out high-quality evidence; combining the evidence with patients and clinical experience to develop optimal rehabilitation interventions measures.4. The best interventions for patients to implement the intervention time of8weeks, the end of the intervention, payment of the relevant scale (HRSD&SS-QOL), the intervention effect evaluation.5. Obtained all the information with SPSS13.0statistical software on the raw data collation and analysis of the mean, standard deviation, t test, variance test.Results:1. Evidence-based rehabilitation intervention program is eight weeks after the implementation of the intervention group after stroke depression in patients with depression, alleviate the extent was significantly higher in patients with post stroke depression. HRSD score (10.37±3.090) in the intervention group compared to interfere with the HRSD score (20.27±7.172) was significantly decreased by t-test, the difference was statistically significant (t=6.056, P<0.001); control HRSD score (17.67±4.999) than the intervention group after the intervention before the HRSD score (19.40±6.770) is slightly lower, but before and after the HRSD score was no significant difference (t=1.142, P=0.263);8weeks after HRSD scores of the intervention group and control group difference was statistically significant (t=6.803, P0.001).2. After8weeks, the evidence-based rehabilitation intervention program implementation, the intervention group, the SS-QOL in each dimension divided equally increased compared with before the intervention are the t-test showed that the energy, family roles, mobility, mood, personality, self-care ability, thinking and statistically significant difference of the scores of the eight dimensions of the upper limb function; the control group in the regular life (only to receive the routine care of the orphanage) after8weeks, the SS-QOL in measured personality, self-care ability, thinking and upper extremity function in four dimensions scores decreased slightly, energy, family roles, language, mobility, emotional, social role, vision and work/labor eight dimension scores slightly higher than eight weeks ago. However, by t-test showed that the SS-QOL in each dimension scores was not statistically significant before and after intervention; analysis of covariance, the intervention group after8weeks in equipartition with the control group, the SS-QOL in dimension, the social roles, vision and the three dimensions of the work/labor in the intervention group score slightly lower than the control group, the other dimensions in the intervention group score significantly higher than that in the control group, language, vision, and work/labor three dimensions, the intervention group and control group statistically significant difference in the other dimension scores.Conclusions:Intervention programs developed for patients with depression in the community after stroke rehabilitation care through evidence-based care, you can improve post-stroke depression in patients with depressive symptoms, alleviate depression, and improve quality of life.
