Objective:Pulmonary embolism is a commonly clinical encountered disease.Due to lack of specific clinical manifestations and signs fields, the patients used to the department of Respiratory Medcine and Cardiolog.For difficult-to-detect and complex clinical manifestations misdiagnosis rate remains high.It’s diversification in clinical examnition and restriction,which brings difficult to the early diagnosis. Here is to review on the methods of Laboratory tests such as plasma D-dimer determination, BNP, Tnl, blood-gas analysis,and the methods of Iconography test such as echocardiography, diagnostic radiology, inspection deep vein, enhanced spiral CT, pulmonary angiography,MRI.In this article we read121domestic case reports about pulmonary embolism to analysis the clinical significance, rationality and feasibility,in order to get a comprehensive understanding of the diagnosis of pulmonary embolism.Methods:We search randomized trials from CBM、CNKI databases,and also hand searching of bibliographies of relevant studies as well as review articles.Then get out the dependable datas from all the literatures which we searched and statistical comput analysis the datas.Result:There are2882PE patients in121case reports about pulmonary embolism,which are finally diagnosis. There were79.88%patients who had used the D-dimer,the ratio of cases with D-dimer greater than500ug/L was90.79%. There were84.25%patients who had used blood-gas-analysis, the hypoxemia Was76.89%by blood-gas-analysis. There were77.00%patients who had used ultrasonic cardiography(UCG),changes detected by ultrasonic cardiography(UCG) was62.19%. There were79.88%patients who had used enhanced spiral CT,arterial embolism in superior pulmonary arteries Was93.47%, There were54.30%patients who had used X-ray, abnormal signs was64.47%.There were6.07%patients who had used pulmonary angiography,and it’s sensitivity is98.87%.Conclusion:1. D-dimer is a high sensitivity test in the assistant diagnosis of pulmonary embolism,it can used for respectively rule out PE in probability patient.2. blood-gas-analysis is another kind of method for bolting pulmonary embolism.3.UCG and DUS can find a part of embolism, which the risk factors of pulmonary embolism.4. enhanced spiral CT and pulmonary angiography can finally diagnosisi pulmonary embolism. In those highly suspected for PE, further evaluations with enhanced spiral CT and pulmonary angiography should be necessary...
