Objectives (1) Based on a constant observation on the hospitalization status of the inpatients diagnosed with traffic accident,violence, fall injury and environment accident in Lanzhou city of Gansu Province from1994to2008. This paper analyzed the trend of distribution of economic burden and harm degree of the disease.(2) This paper also in depth explored and analyzed the influencing factors of economic burden and harm degree of the disease in details in order to closely analyze the ideal approaches to control the overwhelming economic burden of injury patients.Methods The cluster sampling method was conducted to collect medical record data of patients with injury from1994to2008in two provincial general hospital and a emergency center in Lanzhou city. The average hospitalization expenses, potential years of life lost (PYLL), working years of potential life lost (WYPLL), indirect economic burden were used to explore economic burden and harm degree of injury patients. The average growth rate was used to analyze the trend of hospitalization expenses.Results (1) Among four common injuries the proportion of hospitalized men and women were greater than2and the hospitalized cases constitutes of men were greater than60%;15-44age groups were account for60%.Incident of violence in patients were dominated by the25-34age groups into15-24age groups.Except incident of violence,the other there types of injuries are mainly farmers.(2)During15years the average hospitalization expenses increment speed was from10.91%to14.14%and the daily average cost increment speed growed faster was from13.36%to16.03%among four common injuries.Since2002expenses for medicine and operation fee were ranked two first individual charges. Expenses for medicine accounted the proportion of total medical expenses up to40%-50%among four common injuries. Operation fee increment speeds were more than20%.(3)During15years the average hospitalization expenses of traffic accidents were ranked first among four common injuries.From1996to2007the daily average cost of traffic accidents were more than other three types of injuries. Traffic accidents and fall injurys were the average daily hospital costs in the region has growth faster than GDP growth.(4)WYPLL and indirect economic losses per capita of traffic accidents were ranked first among four common injuries in different years. WYPLL of fall injury exhibited an ascend tendency, indirect economic losses per capita of fall injury and injuries from nature and environment exhibited an ascend trend with year period growth,about three times.(5)In different year periods, curative and improved patients were over80%of the total among four common injuries. Admission status of patients for the crisis or emergency hospital ization of patients accounted for more than65%.Conclusions (1)During15years,the number of cases of hospitalized patients showed an increasing trend in Lanzhou city of GanSu Province among four common injuries.Male,15to44age group and the peasants, the students as the high distribution of population of hospitalized patients are the focus groups in the prevention and control of injury.(2)The direct economic burden loaded on the inpatients among the four common jnjuries in Lanzhou city increased al linearly and fastly with time. The patients had to face a large economic burden. Four major injuries per capita cost of hospitalization and the average daily cost of hospitalization annual growth rate is faster.The economic burden of four common injuries in hospitalization patients are more serious.(3) During15years, expenses for medicine which was the first two in the six categories of charges and directly impacted on the severity of medical burden among four common injuries. Operation fee increment speeds were more than20%and after2005the individual charges was over expenses for medicine as the first. Therefore, a high proportion of expenses for medicine and rapid growth of operation fee are the main factors of economic burden for hospitalized patients.(4)The irrational rise of medical cost based on shortening average length of stay only would be unsatisfactory to curb. Lowering geometric mean hospitalization costs per day was also requisite to lower the total economic burden.(5)The incidence of four common injuries are more dangerous with poor prognosis.The cured and improved patients who sented to hospital for treatment were more than80%. With the injury time within24hours back on the passage of four common injuries and improved cure rates in patients with reduced number of cases gradually increased deaths, sent injuries to hospital in time is an important factor influencing the outcome.(6)The injury can be prevented although the risk of injury is serious. Injury prevention and control work need to relevant government departments and the joint participation of all members of society.We can be on the basis of distribution, development of scientific and rational policy measures, rational allocation of health resources to minimize the damage caused by disease burden and economic burden.
