Objective:To investigate the mortality rate, the proportion and distribution of the death of causes in S county, as well as in polluted areas and in control area. Analyzing the major diseases and the burden of disease, comparing the mortality level of polluted areas and control area in order to clarify whether there is a higher incidence of the disease and explore the relationship between water pollution and population health.Methods:The S county was selected for the survey area. According to the environmental test data, the polluted area were villages situated less than 5km away from the river while the control area were villages at least 20km away chosen random by cluster sampling. Retrospective investigation for causes of deaths on residents in S county and in two areas from 2007 to 2009 was carried using scheme of Chinese third national retrospective survey on death causes. Population in survey area was standardized using the national population census data in China in 2000.Results:1. The leading causes of death in S county are cancer, cerebrovascular disease, cardiovascular disease, respiratory system disease and accidental injury. The Top 5 potential years of life lost (PYLL), which followed by accidental injury, cancer, cerebrovascular disease, cardiovascular disease and respiratory system disease.2. The mortality rate in polluted area is higher than that in control area, and the difference was significant. The mortality rate of male and female in polluted area are higher than that in control area respectively. In the leading 5 causes of death in polluted area and control area, the first leading cause of death is cancer, and the second to fifth leading causes of death are cerebrovascular disease, cardiovascular disease and respiratory system disease and accidental injury in proper sequence. Mortality rate in leading 5 causes of death in polluted area are higher than that in control area, and the difference was significant. 3. The top 5 causes cancer death in polluted area are followed by lung cancer, liver cancer, esophageal cancer, gastric cancer, colorectal cancer. But The top 5 causes cancer death in control area are lung cancer, liver cancer tied for second with gastric cancer, esophageal cancer, colorectal cancer. The cancer mortality rates of lung cancer, liver cancer, colorectal cancer in polluted area were significantly higher than that in control area.4. In polluted and control area, the major cause of death is cancer for the 55 to 74 age group. The mortality rate in polluted area is significantly higher than that in control area.Conclusion:As the results revealed, the mortality level, the proportion and distribution of the causes of death and the burden of disease in S county along the SY River were acquired. The mortality level in polluted area is higher than that in control area, where the caner is the major cause of death, which endangers the health of residents seriously, especially for male. Compared to the control area, the cancer and cerebrovascular disease have a trend of younger in polluted area. The increasing mortality rate and the higher mortality level of cancer and cardiovascular disease maybe related with the water pollution in SY River.
