Research On The Current Situation And Countermeasures Of Township Hospital's Medical Service Quality Regulation In Henan Province

ObjectiveThrough the research on status of regulation on Township hospitals'medical quality in Henan Province, the paper means to find and analyze the problems available in the study. At last, combined with the enlightenment that theory of regulation brings, the paper will give out suggestions for improving the regulation and recommendations for national Health reform decision-making reference.MethodsInformation in the study is collected from related literatures that are accessible in database, expert interviews, the questionnaire survey data, interviews, files and work records of regulation and other means. Quantitative data is analyzed with methods of normative analysis and logical reasoning, combined with theories of public interest theory, regulatory capture theory, public choice theory and governance theory.Results(1)Government is the only effective regulatory body, but there are still many problems: lack of manpower, regulatory standards is not scientific, regulatory scheme is poorly enforced, regulatory mode is unreasonable selected, inadequate use of regulation conclusions, Insufficient information disclosure, Lack of coordination among every regulatory bodies.(2)The trade association is weak, independent and it did not play a regulatory role actually.(3) The patients'regulatory initiative were not strong, channels were not enough, so there is a big gap between the patients'regulatory will and their regulatory capacity.(4)In the context of current backward regulatory system, self-regulation of township hospitals plays an important role to ensure the medical service quality, but still it is not enough.ConclusionThere is still space for regulation of township hospital's medical service quality to improve in Henan Province. Through analyzing reasons of the problems lying in the current regulation system and mechanism, combing the inspiration of regulatory theories, the paper gives out suggestions as follows:(1) For the problems that lack of government regulatory capacity, the agency risk and the not-separated regulatory and held system, the study proposed to build a government purchasing the assessment service of medical service quality model, in it, the government will take measures with the help of assessment report, and pay salary for the assessment service provider who should be a hospital or experts after evaluating their work.(2) Improve the regulatory system, foster the regulatory bodies as cultivating industry association, establishing patient organizations and employing medical service quality coordinate regulators. And establish interconnection and coordination mechanism between the regulatory bodies.(3) Design innovative regulatory mechanisms, to ensure equity, transparency, accountability of regulatory activities through the construction of "problem - feedback -improve" model.(4) For the unscientific regulatory standard, the study means to collect suggestions from all the related parties, and gives out the content that should be available in the standard, also the discipline that should be followed in the design of regulatory indicators.(5) For the opaque of government regulatory activities, the study means to introduce third-party monitoring system.Besides, the study gives out suggestions like increasing government investment to regulatory bodies and township hospitals, introducing competition mechanism by allowing private fund to conduct medical institution to provide power for township hospitals'self-regulation.
