Objective:To investigate the general state of health,the clinical manifestation, diseased region and pathologic feature of the primary duodenum carcinoma and how to elect the auxiliary examination,explore the misdiagnosis causes,early diagnosis measures and treatment.of the disease.Methods:A total of 141 cases of primary duodenum carcinoma in the Second Affiliated Hospital of Nanchang University during January 1 2005 to August 31 2010 were retrospectively reviewed.Results:Clinical manifestation was non—specificity. They could be jaundice, abdominal pain,abdominal distention, emesis and bowel obstruction,lump or upper gastrointestinal bleeding. The average internal from onset to being diagnosed was 4.3 months. The tumor was predilection in the descending part of the duodenum,101 cases, whlie 28 in bulbar zone,7 in bulbar-descending boundary;3 in the horizontal part and 2 in the ascending part. The main pathological type is adenocarcinoma, 96.5%.Duodenoscope including ERCP is the highest auxiliary examination,its detection rate was 98.0%, the next was MRI(MRCP) 92.3%, alimentary canal barium meal 84.8%, CT74.0%,gastroscope69.7%, tumor marker CA199 64.3%, B ultrasound 37.1%, tumor marker CEA 32.9%.We should bind these auxiliary examinations exactly to raise the diagnostic rate.Conclusion:(1) There are lack of specificity in the primary duodenum carcinoma, which is high malignancy, misdiagnosis rate and aggravation. It is progression when were final diagnosed (2) It is helpful for us to early diagnosis by improving recognition, reconstructioning pathobiopsy of duodenal and early bind some auxiliary examinations exactly in the primary duodenum carcinoma.
