BackgroundOral lichen planus (OLP) was an un-normal cornification mucosa disease with chronic shallow inflammation, which can happen in oral mucosa or skin, alone or simultaneously. The attack rate of OLP was about 0.51%,Reported in the literature about 0.4%to 10%of carcinogenesis rate. Where the incidence of oral squamous cell carcinoma accounted for an important proportion of the cancer, studies have reported that cancer rates in nearly a decade of 1%-10%.The World Health Organization (WHO) had regarded OLP as precancerous condition. Oral Squamous Cell Carcinoma (OSCC) was the commentest malignant carcinoma in the oral cavity, in which constituted above 90%,and constituted 3%-5%in the whole body. In China, oral and maxillofacial malignant squamous cell carcinoma are the most common, accounting for 80%or more.Tumors, development and transfer is a multi-stage, many factors and affected by many kinds of gene and regulation of the complicated process of factor, its molecular biology is based on oncogene activation and tumor-suppressor gene inactivation.There are a variety of genes related OSCC, including tumor suppressor gene P53, MDM2, PTEN oncogene also has a close relationship with them.P53 gene families, as the most in-depth study of tumor suppressor genes,is play an important role in cell cycle regulation, cell growth inhibition, tumor cell apoptosis induction of tumor and treatment.The deletion or inactivation of P53 gene is closely related to the development of 50%of human tumor. MDM2 is a highly amplified oncogene in many human tumors, such as lung, breast and colon cancer, there are the expression of MDM2 gene amplified. PTEN is the first gene we found which had phosphatase activation, through the completion of phosphatase activity to inhibit tumorigenesis. At present, few studies on the expression and correlation of P53, MDM2and PTEN in OLP and OSCC was repoted.ObjectiveThis topic immunohistochemistry observed the expression of P53,MDM2 and PTEN protein in human normal oral mucosa,OLP and OSCC, and explore the interaction and clinical significance of these proteins in them,then offer diagnostic criteria for cancerous potential monitoring.Materials and methodsIssued in reference to WHO 1996 criteria, all the specimens were collected formJ anuary 2008-December 2009 the pathology archive files of the department of pathology of ZhengZhou University and the biopsy and operation in the department of Stomatology. These specimens included 12 cases of specimens from normal oral mucosa were taken as control, erosive-type 20 cases, non-erosive type 29 cases, all in 49 cases of OLP and 41 cases of OSCC.41case of OSCC (13case of OSCCâ… ,12case of OSCCâ…¡and 16ase of OSCCâ…¢).Immunohistochemical detection for P53,MDM2 and PTEN protein were carried out for all specimens. Calculated the positive cell countings of P53,MDM2 and PTEN.All dates were statistically analysed.Result1. MtP53 protein expression of NOM in the mostly negative, The positive rate of P53 protein in NOM,OLP and OSCC were 16.6%,71.4%,90.2%, showed a positive rate of the gradual increasing trend,The expression intensity of each group were statistically significant (P<0.05).eOLP expression was slightly lower than neOLP, no significant difference between the two groups was statistically significant (P> 0.05).Different grade in OSCC were significant differences between (P<0.05)2. MDM2 protein was not expressed NOM,In the OLP showed weak positive expression in the different pathological grading of OSCC positive rates were higher. The positive rate of MDM2 protein in NOM OLP and OSCC were 0% 10.2%, 80.5%. Expression of each group were statistically significant (P<0.05).Expression in the two types of OLP was no significant difference (P> 0.05). Between the classification in the expression of OSCC was significantly different and the Significant difference (P<0.05).3. The positive rate of PTEN protein from NOM, OLP and OSCC tissue loss rate was 91.7%,87.8%,56.1%. PTEN protein expression in the course of the disease intensity decreased expression intensity of each group were statistically significant (P <0.05). Differences between different groups were statistically significant (P<0.05).4. MtP53 protein and MDM2 protein expression correlation:MtP53 protein and MDM2 protein expression in different squamous cell carcinoma was a significant positive correlation. (r=0.620,P=0.001<0.05)5. MDM2 protein and the relevance of PTEN expression:MDM2 protein and PTEN protein expression rate was significantly negatively correlated (rs=-0.922, P=0.009<0.01). MDM2 protein expression and PTEN-negative expression rate was significantly positive correlation. (rs=0.938,P=0.006<0.01)6. MtP53 protein and the relevance of PTEN expression:PTEN protein was gradually decreased and the rate of malignant oral mucosa showed a significant degree of negative correlation (r=-0.943, P=0.0050.01). PTEN protein-negative rate and gradually increased the positive rate of MtP53 and malignant oral mucosa was significantly positively correlated.(r=0.943,P=0.005<0.01)Conclusion1. P53, PTEN protein is an important indicator of early detection of cancer. Occurrence in OSCC and development, PTEN protein loss and the positive expression of mutant P53 has some synergies.2. MDM2 was over-expression in OSCC, help in evaluating the malignant potential of OLP and the malignant degree and prognosis of OSCC. OSCC and OLP occurrence and development of cancer and are closely related.3. P53, MDM2 protein is OLP and OSCC development and progression of the important molecular event during carcinogenesis in OLP there is mutual regulation4. PTEN and MDM2 gene abnormalities in both are associated with OSCC development process. |
