Objective:To investigate the expression of P-gp protein and in brain of refrectory epilepsy model,and explore whether topiramate(TPM) affect express of P-gp and NF-κB .To explore the role of P-gp and NF-κB resistance mechanisms of refractory epilepsy.Method:Animal modles of focal epilepsy were constructed by chemical kindling (injection of KA to hippocampus).All 84 Wistars Were randomly divided into for three groups.comtrol group(24):N.S injection group KA (30): only KA injection ,group TPM:KA injection with TPM given ,TPM were given oce every day altogether 14 days,TPM were given two hour before KA injection in the 14th weather.After kindling ,the epileptic performance of rats were sacrificed at 6hours 24hours,3days after KA injection respectively. The expression of P-gp and NF-κB of brain tissue were examined by immunohistochemistry. All data of different group were analyzed. Result:1.Epilleptic performance :All rats injected KA had ppileptic performance. Rats of group KA had longer time and higher Racine grade than those of group TPM.2.Expression of P-gp protein and nucler transcription factor–kB in the hipponcampus were increased significantly in the KA group and TPM group compareed with control group(P<0.05).KA group and TPM group had no significant difference(P>0.05).Compared with control group immunohistochemistry experiments found that the expression of P-gp was increased 6 hours after injection of KA (P<0.05),and it was significant increased at 24hours (P<0.05),and it was desended 3days after KA injection (P<0.05).The expression of NF-κB was significant increased 24hours and 3 days after injection of KA compared with 6hours(P<0.05),but 24hours and 3days had no difference(P>0.05).Conclusion: P-gp protein play an important role in the multidrug resistance of refractory epilepsy. Correlation of NF-кB and P-gp suggests they co-affect in the multidrug resistance of refractory epilepsy.TPM does not increase P-gp expression, P-gp over- expression may be only the result of epilepsy paroxysm.
