Hemiarthroplasty Between The Treatment Of Elderly Dhs Fixation Of Unstable Intertrochanteric Fractures Compared

Objective:To compare the clinical effect on the treatment of unstable intertrochanteric fracture in the eldly between the prosthetic replacement and the DHS fixation,and to approach its indication and methods.Method:49 cases were unstable type according to Ewans typing system,and they were followed up from 3months to 2year.21cases of all had been treated with bipolar prosthetic replacement,and were more than 70 years old. 28 cases of all had been treated with DHS fixation,and were72.4years old in average. Record the operation time,amount of bleeding of two groups,and followed up how long the patients get off bed,and the occurrence of complicating diseases; all of the cases were evaluated by Harris grading system,and compared those index bween two groupes.The data statistics withSPSS13.0.Result :there was no statistics differenee in oPeration time,amount of bleed and complications incidence rate. The replacement got off bed earlier than fixation group.but the complicates incidence of fixation group are more than replacement group because time of bed was longer.The excellent rate of replacement group is better than fixation group,but there no difference instatistic.Conelusino: The prosthetic replacement can be one kind of treatment to the unstalble intertrochanteric fractures in the eldly,but we should master the operation indication strictly,and the propotional treatment according to its characteristic shou1d be take to reduce the occurrence of complications..Because the cases of particular disease were few,it was limited to some extent to reflect the result accurately and overall. According to the result of the investigation and the abstract superiority of the prosthetic replacement,It is possible to perform the freely control study in large scale between in the prosthttic replacement group and the internal fixation group.
