| Research purposes:Through the comparative analysis of the clinical therapeutic effect of proximal femoral nail antirotation and artificial femoral head arthroplasty to elderly osteoporotic femoral intertrochanteric fracture,we find the proper way to cure the elderly osteoporotic femoral intertrochanteric fracture.Research methods:From June 1st of 2006 to June 1st of 2014,the Osteopedic Ward of Rongcheng people’s hospital had 240 femoral intertrochanteric fracture admitted,among of which 158 cases were elderly osteoporotic femoral intertrochanteric fracture.In all the elderly osteoporotic femoral intertrochanteric fracture,66 cases were male,and the other 92 cases were female.The age of them were between 65 years and 95 years,with the average age 74.2 years.The total patients showed the visible osteoporotic presentation through X ray scan or the bone mineral density examination.According to the Singh Index,all the patients were below level IV,with the diagnosis of osteoporosis.All the patients were allocated into two groups randomized,group I were treated with proximal femoral nail antirotation and group II were treated with artificial femoral head arthroplasty.Group I had 83 cases,among of them 32 cases were male and the other 51 cases were female,with the average age of(75.36±4.86)years,while Group II had 75 cases,among of them 30 cases were male and the other 45 cases were female,with the average age of(75.19±5.37)years.To comparatively analyze the clinical therapeutic effect of the two different methods to the elderly osteoporotic femoral intertrochanteric fracture,we had the following research items recorded,such as preoperation demographic information including age,gender,injured side,fracture AO type,injury mechanism,ASA classification,the motility before injury,following time,intraoperation and postoperation related index,such as length of incision,operation time,intraoperative blood loss,fluoroscopy time,postoperative drainage,hospitalization time,the time of weight loading after operation,early complications such as incision infection,bed sore,deep venous thrombosis,pneumonia,urinary infection,death,and hip function Harris score.Research results:Through the recorded preoperative demographic information such as age,gender,injured side,fracture AO type,injury mechanism,ASA classification,the motility before injury,following time,we comparatively analyzed the clinical therapeutic effect of proximal femoral nail antirotation and artificial femoral head arthroplasty to elderly osteoporotic femoral intertrochanteric fracture,then we found no statistical difference between the two groups,proving that data of this two groups had comparability.Through the recorded operative traumatic index including length of incision,operation time,intraoperative blood loss,postoperative drainage,we comparatively analyzed the clinical therapeutic effect of proximal femoral nail antirotation and artificial femoral head arthroplasty to elderly osteoporotic femoral intertrochanteric fracture,then we found that proximal femoral nail antirotation was better than artificial femoral head arthroplasty,and the difference had statistic significance.While the difference between the the clinical therapeutic effect of proximal femoral nail antirotation and artificial femoral head arthroplasty to elderly osteoporotic femoral intertrochanteric fracture in the operative fluoroscopy time and hospitalization time showed that artificial femoral head arthroplasty was better than proximal femoral nail antirotation,and the difference had statistic significance.The difference between the the clinical therapeutic effect of proximal femoral nail antirotation and artificial femoral head arthroplasty to elderly osteoporotic femoral intertrochanteric fracture in the time of weight loading after operation showed that artificial femoral head arthroplasty was better than proximal femoral nail antirotation,and the difference had statistic significance.Through the recorded early complication index including incision infection,bed sore,deep venous thrombosis,pneumonia,urinary infection,death,we comparatively analyzed the clinical therapeutic effect of proximal femoral nail antirotation and artificial femoral head arthroplasty to the elderly osteoporotic femoral intertrochanteric fracture,then we found that artificial femoral head arthroplasty was better thanproximal femoral nail antirotation,and the difference had statistic significance.Through the recorded following hip function Harris score of one year after operation,we comparatively analyzed the clinical therapeutic effect of proximal femoral nail antirotation and artificial femoral head arthroplasty to the elderly osteoporotic femoral intertrochanteric fracture,then we found that artificial femoral head arthroplasty was similar to proximal femoral nail antirotation,and the difference had no statistic significance.Research conclusion:Artificial femoral head arthroplasty treating the elderly osteoporotic femoral intertrachanteric fracture could give patients the opportunity to get weight loaded earlier,at the same time,it could reduce hospitalization time,then make early staying in bed complication less,getting satisfactory function.Artificial femoral head arthroplasty could be one effective and selective solution to treating elderly osteoporotic femoral intertrochanteric fracture. |
