Since reform and opening up, the health care system of China gradually facing unprecedented challenges, including rising medical expenses, medical and health system efficiency of slow growth, unequal distribution of health resources and so on, which has become urgent Social problems need to resolve.In the health care system reform process of China, the development of community health services is considered an important breakthrough in the effort in promoting the equity of health resources. The purpose of development of community health services is to establish a new urban health service system based on the community health service system, which seeks to optimize the medical and health resources, enhance the health level of health care, and implement the public welfare of health services, by provide comprehensive, economic, effective and continuous basic medical services.Starting from September of 2005, Changning District of Shanghai initiated a new round community healthcare reform and functioned as an experimental unit. This reform is characterized as a totally new service pattern, with the genetal practitioner team (GP Team) as basis. According to the team theory, the mode of team-work adapts to community health services better, can produce higher efficiency, and can achieve the goals of community health services better. Therefore, the operating efficiency, effectiveness and the adaptation for the reform of the GP team generate great influence on the development of community health services and the effectiveness of the reform.The purpose of this thesis is to discuss how the general mode of the GP team function and achieve the goal in practice. GP teams in Changning District in Shanghai have been studied in this thesis. Information and data was collected through relevant literatures, questionnaire surveys, interviews and other methods, by which the operational conduct, efficiency rating, function and the satisfaction of residents and staff was analyzed to assess the performance of the GP Team. The effectiveness of the GP team was also studied from the work design, team consists, outside conditions and process factors, based on the team effectiveness model.This thesis is divided into five chapters. In the first chapter, the background of the development of community health services was described. The research purposes and study method of this thesis was also introduced in the chapter. In the second chapter, the concept of community health services, the results of previous research and contributions of predecessors was described. Then the team effectiveness theory and motivation theory was introduced, which is the basis of the study in this thesis. In the third chapter, the establishment and function of the GP Team was introduced, and then the performance of the GP Team was analyzed by the date collected. In the forth chapter, the effectiveness of the GP Team was studied based on the team effectiveness model.At last, the study results were summarized and some suggestions were given in the fifth chapter. This thesis has got the conclusion that the work of GP Teams in Changning District has achieved an effective integration of medical resources and the transfer of the focus of work, and has established a comprehensive and continuous working model. The GP Teams have also provided a smooth increasing, effective and fruitful medical service, got a better social benefits and higher resident satisfaction. However, according to the study result based on the team effectiveness model, there is still a considerable gap between GP Teams in Changning District and high efficiency team in theory. And it is necessary to take improving measures on the ability and structure of the GP Team, the working mode, external support and the member's adaptation of the team work, to enhance the general effectiveness of the GP Teams. At the end of this paper, some improving suggestions were given for the GP Team, including enhance the ability of the GP team, build a good culture atmosphere, provide supporting policy and human resource purvey, establish a comprehensive motivating system, and optimize the team working mechanism.
