This thesis includes two parts, one is clinical study, the other is literature summary.ⅠClinical studyObjective: The clinical study discusses the chronic prostatitis(CP) TCM syndromes indistribution, composition characteristic, differentiation of symptoms's important elements andthe feature of evolution, mainly through the epidemiology investigation to 278 CP patients,which provides the basis for the CP clinical differentiation of symptoms.Methods: The general data means and the individual questionnaire inventory weredesigned by us, which contained detailed content and relevant factors with CP. We know theCP TCM syndromes in distribution, composition characteristic, differentiation of symptoms'simportant elements and the feature of evolution mainly through Cross-sectional Study of theepidemiology investigation. At the same time, we collect the urinary tract,radiating pain andgeneral symptom and tongue demonstration of the TCM chief syndromes and importantelements, then according to their frequency analyze in statistics. The initia approach the CPTCM chief syndromes in differentiation of symptoms's important elements and the feature ofevolution.Results: It was age,occupation,incidence,course of disease and the factors of induce oraggravate that faily influenced the CP patients, these factors have epidemiology feature. Forexample, Young people,drivers,palindromia patients,course of disease from 3 to 6 monthsand infextation existed. In the aspects of CP TCM syndromes in distribution, compositioncharacteristic, differentiation of symptoms's important elements and the feature of evolution,syndrome of stagnation of liver-QI holds absolute superiority, and with dampness-heat,blood stasis to constitute the CP TCM chief syndromes and important elements. The resultshows that the main CP TCM syndromes are stagnation of liver-QI,dampness-heat,bloodstasis,deficiency of kidney-YANG and hepatic and renal yin deficiency, but we can't collectsole syndrome of deficiency of kidney-YANG and hepatic and renal yin deficiency.symptoms's elements have clinical significance in CP's classification of NIH (NationalInstitutes of Health). In the model ofⅡ, dampness-heat is most common; in the model ofⅢA, stagnation of liver-QI holds important places; in the model ofⅢB, stagnation ofliver-QI and blood stasis are not only mostly, but also transmigrate and combined, deficient syndrome's frequency of occurrence is more than model ofⅡandⅢA. The syndrome ofstagnation of liver-QI is the key of the evolution of syndromes, the hinge of the transformingof pathogenesis. Stagnation of liver-QI often induces deficiency of kidney-YANG andhepatic and renal yin deficiency and blood stasis. The five syndromes and importantelements are time-dependent. Stagnation of liver-QI syndrome is related to the factor ofpsychological factor. So it often runs through the whole course of CP once it appears.Theblood stasis and other minor syndromes are only the pathogeny of some phase in of thedisease. Syndrome differentiation by urinary tract symptom and alleviate radiating pain isthe main determining factor to differentiation of syndromes of CP.Conclusion: Firstly, The syndrome of stagnation of liver-QI is the key of the evolution ofsyndromes. Secondly, The five syndromes and important elements are time-dependent. Thefamiliar beginning syndrome of CP is dampness-heat. The longer the course of the illness, themore syndrome occour, for example, deficiency of kidney-YANG and hepatic and renal yindeficiency and blood stasis. Thirdly, It also indicates the TCM syndromes of CP arecomplicated. Various syndromes always transmigrate and combined. They are unable to beseparated clearly. The sole TCM syndrome can not reflect the process of pathogenesischanges in the progression of CP by far.The longer the course of the illness, the more relapse the condition is. So we can notreplace differenriation of syndromes by simple typing. Differentiation of syndromes must bebased on the the essence of syndrome and syndrome evolution. Though the treatment must aimat the urinary tract symptom and alleviate radiating pain as soon as possible, the whole bodysymptoms should not be neglected.ⅡLiterature summaryThe literature summary title is the status and progress of CP TCM syndromes about therule of evolution. This literature summary in which a large number of documents about therule of evolution of CP TCM syndromes.
