Objective: To observe the changes of serum leptin in patients with coronary heart diseases (CHD) and coronary heart diseases with type 2 diabetes mellitus (T2DM); to analysis the correlation between serum leptin (LP) and high sensitive C-reactive protein (Hs-CRP), uric acid (UA), the blood fats, and so on; to disscuss the clinical value of LP in CHD and CHD with T2DM in order to understand the effect of LP in CHD and CHD with T2DM.Methods:Choose patients with CHD in department two of Vasculocardiology and normal controls in physical examination center, hospital affiliated to Yan'an University from July 2009 to January 2010, total 84 examples. CHD with T2DM group is 32 examples, CHD without DM group is 33 examples, and normal control group (NCG) is 19 examples. Concentration of serum LP, Hs-CRP and UA were assayed by enzyme linked immuno sorrbent assay (ELISA). All statistical analysis was performed by SPSS17.0 software.Results: 1. Three groups (CHD with T2DM group, CHD without DM group and NCG.) of crowd common material comparison: the mean value of the body mass index (BMI) and waist hip radio (WHR) in the CHD with T2DM group are higher than in the CHD without DM group and NCG (P<0.05, p<0.05), and which in CHD without DM group and NCG have not notable difference (P>0.05); the mean value of the triglyceride (TG) and total cholesterol (TC) levels in NCG are lower than in the CHD without DM group and the CHD with T2DM group (P<0.001, P<0.05), and which in CHD without DM group and the CHD with T2DM group have not notable difference (P>0.05). 2. Three groups (CHD with T2DM group, CHD without DM group and NCG.) of crowd serum LP , Hs-CRP and UA comparison: the mean value of the serum LP and Hs-CRP in the CHD with T2DM group are hihger than in the CHD without DM group and NCG (P<0.001, p<0.001), and which in CHD without DM group and NCG have not notable difference (P>0.05); the mean value of the UA have not notable difference in the three groups (P>0.05). 3. CHD patients were divided into three groups according to the stability of plaque, which is SAP, UAP and AMI; then make comparison of the three groups of crowd the serum LP, Hs-CRP and UA: the mean value of the serum LP in AMI group is higher than SAP group (P<0.05), and that in UAP group and AMI group has not notable difference (P>0.05), and that in UAP group and SAP group has not notable difference too (P>0.05); the mean value of the Hs-CRP in AMI group is higher than UAP group (P<0.05), and that in UAP group is higher than UAP group too (P<0.05), and that in AMI group is significantly higher than SAP group (P<0.001); the mean value of the UA have not notable difference in the three groups (P>0.05). The Spearman rank correlation analysis presents that the serum LP and Hs-CRP level are positive correlated with the stability of plaque (r=0.254, P<0.05; r=0.486, P<0.05); the serum LP is positive correlated with the serum Hs-CRP and UA (r=0.527, P<0.01; r=0.304, P<0.05).Conclusion: 1. LP may play an important role in the occurrence and development of CHD and DM through inflamrnatory reaction. 2. The levels of serum LP rise gradually with the stability of plaque changed; the level of serum LP is an important biochemical indicator to reflect the stability of plaque. 3. LP is closely related to obesity, and the patient with obesity is often accompanied with hyperleptinemia.
