Objective:To evaluate the effect of bone graft in combination with splint-like PFM bridge treatment on conserving loosened disal teeth of KennedyIII dentition defect. Methods:20 patients with the first molar lost , the loosend disal abutment teeth with periodontal bone defects were selected, and divided into two groups. In the treatment group ,10 patients'loosened disal teeth were treated by bone graft in combination with splint-like PFM bridge. While the 10 patients'loosened disal teeth were treated by splint-like PFM bridge alone in the control group . All the patients were followed up by 6-12 months. Periodontal clinical index( PD,CAL,BI,TM), value of masticatory efficiency were measured, x-ray and photoes were taken before surgery ,3 months,6 months surgery. Results: 1) In the treatment group, a decrease in PD,TM, ME and a gain of CAL were seen following the use of CHA in the distal abutment teeth, and statistical difference was been found between 0/3/6 month(P<0.05). Otherwise , the CAL of the other abutment teeth had no statistical difference between 0/3/6 month (P<0.05). The BI value of all the abutment teeth had statistical difference between 0/3/6 month(P<0.05). Advanced bone loss was not found in any case,partially bone repair was found in all of the loosened disal teeth. 2)No statistical difference was found between treatment group and control group in PD,CAL(P<0.05) at the base line. Statistical difference was found in CAL after 6 months between the two groups (P<0.05) .the results indicate that bone graft in combination with splint-like PFM bridge can improve periodontal tissue regeneration, Advanced bone loss is not found in the two groups,partially bone repair was found in all of the loosened disal teeth in treatment group after 6 months. Conclusion:1) The short-term effect of applying bone graft in combination with splint-like PFM bridge on conserving loosened disal teeth have been identified quite satisfiding in this study. 2) On the condition of strict selection of indication, Splint-like PFM bridge is indicated as an effective treatment on conserving loosened disal teeth as well as restoring the missing teeth. 3) By comparing the clinical results of the two groups, bone graft in combination with splint-like PFM bridge is beneficial to accentuating periodontal tissue regeneration of the distal abutment teeth.
