Objective:To study the clinical application and evaluate the clinical effect of combined PFM as fixed splinting prosthesis in patients with periodontal disease.Methods:Choose 12 patients who were diagnosed as patients with mild-to-moderate chronic adult periodontitis in the Comprehensive department and VIP department of the Affiliated Stomatologic Hospital of Zhejiang University from July 2010 to January 2011. Tooth mobility degrees should be less thanⅡ, X-ray showed that the alveolar bone absorption does not exceed 1/2 of the root's length, periodontal foundation treatment or periodontal surgery was performed, and the periodontal status stability should be good. After detailed clinical and radiographic examinations, periodontal therapy and other preparation treatment, including remove the inappropriate prosthesis, endodontic therapy, remove the residual root which is unable to keep. Prepare the tooth according to the requirements of PFM, retract the gingival, then make the impression. As for sesidual roots and crowns, observe a week after RCT, then prepare the roots for metal post-core or glass fiber post, bond the glass fiber post with Rely X Unicem, moulding the shape with composite resin, then prepare the tooth as usual. Design and make the fixed splint. For posterior teeth the edge of prosthesis is designed upon the gingiva edge, for anterior teeth with high aesthetic demand, the edge of prosthesis is designed under the gingival edge of 0.5mm. At last, try on and bond the fixed splint. After three months for follow-ups and evaluations of the clinical effects and the patients' assessment.Result:After three months for follow-ups, most patients had subjective evaluation for good. And the clinical examination showed:①Comparing the postoperative X-ray films with preoperative X-ray films, the level of alveolar shows no progressive absorption.②No loose of the splinting and the abutment teeth.③The BOP before fixed splinting was 17.41±2.97%, three months after fixed splinting, the BOP was17.24±5.74%, P>0.05, no significant differences.④The fixed splints didn't released or broken off during the three months.⑤Secondary caries, broken, dental pulp necrosis, root inflammation does not appear in the abutment teeth during the three months.Conclusion:After completed periodontal therapy, the short-term clinical behavior of fixed splinting for patients with chronic periodontitis is encouraging.
